Dear Mr. Pollack:
Our office has been informed that an unsatisfactory condition may
exist with regards to the hot water heater located at the
above -referenced address. This notice is to inform you that should
you install new equipment, a permit will be required to be
obtained.

the hours of 8:00 am.m. and 4:00 p.m., weekdays.
