This Change Order provides payment for furnishing and installing a monument case, cover and
anchor. The contract included a pay item to adjust existing monument cases to grade. However,
an existing monument case was discovered to be cracked and not re-useable.
The Change order increases the cost of the project $524.55. This project is 86.5%funded by
Federal Grant STPUL -3840 (010) and the 13.5%match is funded by Spokane Valley Street
Preservation Fund 311. There are sufficient funds in the project budget to cover these costs.

8-13 Monument Cases

8-13.1 Description
Add the following:
This work also includes salvaging the existing monument; coring the asphalt or
setting the monument case within concrete pavement; furnishing and placing the
monument case; adjusting the existing monument case; and coordinating with a
City Consultant Surveyor who will re-establish the restored point on the set
monument within the monument case.
8-13.2 Materials
Add the following:
Monument shall be furnished by the City. The case, rod and cover shall be
furnished by the Contractor. The Case and rod shall conform to Spokane Valley
Standard Plan R-145.
8-13.3 Construction Requirements
The last sentence is replaced with the following:
The Contractor shall salvage the existing monument and deliver it to the Engineer.
The Contractor shall coordinate with the licensed Land Surveyor (City Surveyor)
who the City has retained to be responsible for the filing of permits for monument
removal and replacement with the Washington State Department of Natural
Resources, as required by WAC-120-070.
The Contractor shall furnish and install the monument case and the monument
anchor pipe. The Contractor shall set the monument in cement epoxy with a
minimum compressive yield strength of 5,000 psi.
The Surveyor Consultant, hired by the City shall furnish, install, label, and mark
the monument cap inside the monument case. The Contractor shall notify the City
Surveyor at least 7 calendar days prior to the date that the monument case will be
placed.

When surrounded by hot mix asphalt pavement, a new monument case or
adjustment of an existing monument case shall be set after the first lift of paving is
completed. The Contractor shall center the case on the point. The Contractor
shall place the final HMA lift around the case so that the top of the monument
cover shall be recessed below the surrounding pavement elevation in accordance
with Section 5-04.3(13).

When surrounded by Portland Cement Concrete pavement the monument case
shall be placed in the formwork prior to placing the concrete. The top of the
monument cover shall be recessed below the surrounding pavement elevation in
accordance with Section 5-04.3(13).

1. Describe the Change This Change Order pays for furnishing and installing a
monument case, lid and anchor rod per WSDOT Std Drawings.
2. Evolution of the Change
The Contract plans call for adjusting existing monument cases to finish grade. An
existing monument case near Sprague Ave. Station 131+45, 0' Rt was found to
be cracked and not re-useable. This change order will pay for a new monument
case to be installed.
3. Payment The $524.55 cost was determined by adding the $149.55 cost of the
monument case to the existing $375 bid item price for monument adjustment.
4. Time No additional time was granted or taken away.
5. Prior Approval
The Change Order was signed by the acting Public Works Director August 29,
2014.
