1.Convene Meeting: Tom convened the meeting at 3:09p.m.
2.Minutes of September 4, 2013: Consensus to approve the minutes as presented.
3.Accident Prevention Plan: Discussion re: Argus Plan and software used to create city-wide plan

The question arose, do we include in our program how to enter a confined space when

John contacted WCIA and they stated it was best to not
o
rather include how to recognize a confined space and what to do if an emergency happens
concerning a confined space
Michelle stated that Mike Jackson noted the Argus plan was specific to PW and not the entire
o
city; it would be good to obtain a list of all confined spaces and find out if, for example, do
building inspectors enter confined spaces
Jay said they enter crawl spaces and attics
o
Team will provide a list to Michelle by middle of December from discussions with
o
their departments as to what may be a confined space entry pertaining to their
line of work
4.Safety Concerns:
John Whitehead stated the City had a claim for a slip and fall at the Precinct
A Sgt. saw a woman slip on a rug that slipped on the tiled floor
Brian from Parks reviewed the floor and felt the rugs were ok
John asked for input
Chris will reassess

Contact the company who cleans/replaces the rugs; Jay asked if the company notes whether
o
the rugs are non-skid or not
If not, perhaps they have rug grippers to place under them
o
5. Other Business:
Tom attended a Safety Conference where they stated an OSHA injury log is supposed to be posted
John said he does post it and it is a posting period of Feb April he believed, but he left it up all year
John will get a copy to Tom to post at the Maintenance shop next year
There being no further business, the meeting was adjourned at 3:40p.m.
