At a minimum an inspection should be requested at !east once every 180 days to assure the validity of the permit. A permit may be renewed within one year of the date 01 expiration for one-half the original fee, subject to certain limitations—please call us if you have any questions. All refund requests must be made in writing by the applicantlpermittee (shown on the reverse) no later than 180 days after the date 01 fee payment.
