Envlronmental Checklist File No. Pu►p4se of CAecklist: The State Environcnental Poticy Act (SEPA) chapter 43.21C RCW, requires all goverrxnental agentles to consider the enviroranental iiapacts of a proposal berore naking Aecisions. An Environmental Impact Stateaent (EIS) must be prepared for all proposals with probable signtficant adverse inpacts on the quality of the envirorment. The purpose of this checklist is to provide information to help you and the agency ldentify tmpacts from your proposal (and to reduce or avoid irtQacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. Instructions for Applicants: This enviroru.ental checklist asks you to describe scne basic information aba t your proposal. Governmental agencies use this checklist to determine whether the enviranmental impacts of your proposal are significant, requiring preparation of an EIS. Answrer the Questions briefly, Nith the most precise infonnation known, or give the best description you can. `lou must answer each question accurately and carefully, to the best of your knowiedge. In most cases, you sfiould be able to answer the questions fron your own observations or proSect plans without the need to hire experts. If you really do not know the answer, or if a question does not apply to your proposal. write 'do not know' or 'does not apply.' Complete answers to the questlons na+ may avoid unnecessary delays later. Sorae questions ask about governmental regulations, such as zoning, shoreline, and landmark destgnations. Answer these questions if you can. If you have problems. the governmental agencies can assist you. The checklist questions apply to all parts of your proposal. even 1f you plan to do thern over a pertod of time or on different parcels of land. Attach any additional inforn►ation that will descrlbe your proposal or 1ts environrtental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional info nnation reasonably related to deternining if there may,be significant adverse impact. Use of checklist for nonproject proposals: Complete this checklist for nonpro3ect proposals, even though questions may be ansxered 'dces not apply'. IN AODITION, ca.iplete the SUPPLE?IENTAL SHEET FOR NONPROJECT ACTIONS(Part D). For nonpro3ett actions, the references 1n the checklist to the words "project,' 'applicant,' and 'property or site' should be read as 'proposal."
