Dear Mr. Higgins:
In accordance with WAC 246-215, as referenced in Chapter 8 part 2 of the Washington State Retail Food
Code Working Document, this agency has reviewed the food menu, the food preparation sheet, and the
building construction plans for the above-referenced project. We have found the plans to be incomplete
and are not approved for the following reasons:
1. Complete and submit "Plan Review application and checklist"-attached.
2. Provide an updated menu and preparation steps for items on the menu.
3. Provide a site plan.
4. Provide floor plan with equipment schedule (include manufacturer and model number), sinks
and plumbing plan, and finish material schedule-see attached example. Note-espresso
machine, ice maker and bins, 3-compartment sink and dishwashers must be indirectly drained.
5. Include list of light fixtures- must be covered or non-shatter bulbs in food prep areas.
6. Provide information on garbage disposal.
A fee of$110.00 (for 2012) or$120.00 (for 2013) per hour is charged for plan review and pre-occupancy
inspections. Enclosed is an invoice for the plan review. The fee is to be paid within 30 days of receipt of
invoice. An invoice for the pre-occupancy and any additional plan review fees will be given to the
person in charge at the time of the pre-occupancy inspection. A $110.00 late fee will be applicable if
the plan review and pre-occupancy fees are not paid within the 30 days.
Enclosed is an application for Plan Review; please complete and return with the items listed above.
Applications(s) are to be submitted at least fourteen (14) days prior to the projected date of opening
to avoid paying a double permit fee. No applications(s)for permits)shall be accepted less than three
(3) days prior to the date of opening due to the time needed to review the application. The fee must
be paid prior to scheduling a pre-occupancy inspection. Payment can be made over the phone with a
Visa, MasterCard, or Discover by calling(509)324-1560 ext. 2.
After you have completed construction, the Health District will conduct a pre-occupancy inspection. At
the time of this inspection it will be verified that the construction is in compliance with the approved
plans. (Any changes in menu or equipment shall be pre-approved by the Health District prior to
construction.) All food equipment must be certified or classified for sanitation by an American National
Standards Institute (ANSI)-accredited certification program (e.g. NSF), or equivalent. Any unapproved
home-style equipment found on the premises will have to be removed.
The permit to operate will be given to you at the end of the pre-occupancy inspection if the
construction is found acceptable. Contact all the other appropriate agencies for approval prior to
opening. To insure that you obtain your pre-opening inspection in a timely manner we request that
you contact us at least three (3)days before you want an inspection to arrange it in our schedule.
All employees handling unwrapped food are to obtain a food worker card within 14 days of
employment. Food and Beverage training is now available at www.srhd.org/safetv/food.
