You are hereby directed to make the changes listed below in the subject agreement.
Nature of the change: The agreement is modified as follows:
Item 1: Amend the scope of services as described in ARTICLE 20. Scope of Work, Part 2,
Construction Administration Phase, to include developing two change order requests and negotiating a
price with the construction contractor for the following:
1) Revising the layout of the second floor of the senior wing. The revised layout shall include deleting
the walls between rooms 231, 232, 234 and corridor 230 and adding a dance floor to this same
area. The change order shall include all designs, specifications and drawings for architectural,
structural, electrical and mechanical elements.
2) Completing the second floor of the education wing with all necessary interior finishes, electrical and
mechanical appurtenances. -- - .
The Fee for these services shall be on a time and materials basis with a maximum amount not exceed
$17, 500.00.
The changes result in the following adjustment of the agreement price and time:
AGREEMENT AMOUNT
Agreement price prior to Phis change order $076,155.65
Agreement price (increase) resulting from this change order $17,500.00
Agreement price including this change order shall not exceed $693,655.65
