An Environmental Impact Statement (EIS) must be prepared for all proposals with probable -significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. Instructions for Applicants: This environmental checklist asks you to describe some basic information about your proposal. Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the best description you can. You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should be able to answer the questions from your own observations or project plans without the need to hire experts. If you really do not know the answer, or if a question does not apply to your proposal, write "do not know" or "does not apply." Complete answers to the questions now may avoid unnecessary delays later. Some questions ask about governmental regulations, such as zoning, shoreline, and landmark designations. Answer these questions if you can. If you have problems, the governmental agencies can assist you. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. Use of checklist for nonproject proposals: Complete this checklist for nonproject proposals, even though questions may be answered "does not apply". BACKGROUND/ANALYSIS The request was originally scheduled for the October 18, 1979 hearing. It was continued until several environmental concerns were answered. The concerns were traffic generation, sewage disposal, and storm water. At the writing of this report only the traffic question had been addressed. The other two concerns have yet to be resolved between the applicants and the appropriate agencies. IV. AGENCIES CONSULTED/COMMENTS RECEIVED a) COUNTY FIRE MARSHAL 1. The site is located in Fire District #1. 2. Mains and hydrants need to be installed in the above areas. Access plans for firefighting equipment around the building shall be approved prior to building permit issuance. 3. Adequate water for fire protection and distribution of water is required for the following zone changes. A water plan is required to be submitted prior to any development or use of subject lands. b) COUNTY ENGINEER 1. Applicant shall dedicate 5 feet on Herald Road for right-of-way. 2. Applicant shall dedicate 10 feet on Main Avenue for right-of- way. 2 ZONE RECLASSIFICATICIV NUMBER: ZE-155-79 IV. AGENCIES CONSULTED/COMMENTS RECEIVED (continued) 3. Applicant shall dedicate the right-of-way for the extension of Felts Road between Sprague Avenue and Main Avenue as directed by and as shown on the County Engineer's Plat Plan of the proposed development. 4. Applicant shall dedicate a 20 foot radius on Herald Road and Main Avenue. 5. Applicant shall dedicate a 20 foot radius on Felts Road and Main Avenue. 6. Access permits shall be obtained from the Spokane County Engineer's Office prior to issuance of building permits. 7. Applicant shall construct cement concrete curb, sidewalk and pave to existing pavement on Herlad Road and Main Avenue. 8. Special: The construction of Felts Road; reconstruction of existing traffic islands on Sprague Avenue adjacent to the proposed site and modification to the existing traffic signal at Sprague and Felts Road Extension shall be completed by the developer in cooperation with and as directed by the Spokane County Engineer. 9. Appropriate on and off-site drainage and access plans shall be approved by the Spokane County Engineer's Office prior to the issuance of building permits for the project. 10. The design, location and arrangement of parking stalls shall be in accordance with standard traffic engineering practices. Any portion of the project which is to be occupied or traveled by automobiles shall be maintained in hard surface paving. c) WATER PURVEYOR 1. Purveyor is Modern Electric Water Company and they are able to supply the site with adequate water for domestic fire and irrigation uses. d) COUNTY HEALTH DISTRICT 1. No comments received. e) SEPA 1. That the provisions of SEPA's Notice of Action pursuant to 43.21C.080 RCW be initiated by the applicant within thirty (30) days of final action of the Responsible Administrative Body and be completed before any site improvements are made. 3 ZONE RECLASSIEICATI"N NUMBER: ZE-155-79 2. A water plan is required to be submitted prior to any development or use of subject lands. b) COUNTY ENGINEER 1. Applicant shall dedicate 5 feet on Herald Road for right-of-way. 2. Purveyor is Modern Electric Water Company and they are able to supply the site with adequate water for domestic fire and irrigation uses. d) COUNTY HEALTH DISTRICT 1. No comments received. e) SEPA 1. That the provisions of SEPA's Notice of Action pursuant to 43.21C.080 RCW be initiated by the applicant within thirty (30) days of final action of the Responsible Administrative Body and be completed before any site improvements are made. 3 ZONE RECLASSIFICATION NUMBER: ZE-155-79 2.
