I Introduction: The State Environmental Policy Act of 1971, Ghaptei- 43.21C, RC-VV, requires all state and local governmental agencies to consider environmental values both for their own actions and when licensing private proposals, The Act also requires that an Environmental Impact Statement be prepared for all major actions significantly (and "adversely", as per WAC 197-10) affecting the quality of the environment. The purpose of this checklist is to help the agencies involved determine whether or not a proposal is such a m-ajor action. Please answer the following questions as completely as you can with the information presently available to you. Please answer questions as "yes" or "maybe" if, in your opinion, even only slight impacts will result. The reviewers of the checklist will be aware of and concern themselves with the decrree of impact, asking you for more informa- tion, if necessary. Where explanations of your answers are required, or where you ~ believe an explanation would be.helpful to government decision-makers, include your explanation in the space provided, or use additional pages, if necessary. You should include references to any reports or studies of which you are aware and which are relevant to the answers you provide. Complete answers to these questions now will help all agencies involved with your proposal to undertake the required environmental review cvit►iout unnecessary delay. The following questions apply to your total proposal, not just to the license for which you are currently applying or the proposal for which approval is sought. Your answers should include the impacts which will be caused by your proposal when it is completed, even though completion may not occur until sometime in the future. This will allow all of the agencies which will be involved to complete their environmental review now, without duplicating paperwork in the future . No application shall be processed until the checklist has been completed and returned to the appropriate County department. State law requires explanations for every "yes" and "maybe" answer on the checklist. The person completing the form may be required to provide explanations for "no" answers, and in some cases, more detailed information to aid in a threshold determination, NOTE: This is a standard form being useci by all state and local agencies in the State of Washington for various types of proposals. . Many of the questions may not apply to your proposal. If a question does not apply, just answer it "no" and continue on to the next question. 1 ' ~ . . , ENVIRONMENTAL CHECKLIST FORM i , BACKGROUND ~ 1, Name of Proponent: ~4 `4l9 All) YC . Irrigation requires effluent disinfection in most cases These systems also require relatively large areas for year around operation. Land and some operational costs are more than those for septic tanks and drainfields Agaia, without a specific additional treatment to remove diasolved solids or completely sealing the dispusal area from percolation, some dissolved salts will be carried with effluent recharge to the groundwater E. Historically, urban density development has resulted in polluted groundwater beneath the develapment and increased values of land to the extent space for individual sewage dis- posal drainfields is too expensive for that use P' Conclusion (1) The use of subsurface sewage disposal systems for this project will have a negligible impact on the water quality (Z) The development of this project in conjunction with other surrounding urbanizing project can be expected ultimately to have at least a moderate impact on the water qualitq paf * THE EFFECT OF APPLIED SURFACE WATERS ON GROUNDWATER QUALITY IN THE SPOKANE VALLEY David K Todd, Sectioa 608 Water Resources Study - Metropolitan Spokane Region, Corps of Engineera, Department of the Army, Seattle District, April 7, 1975 ~E C~f a r 1 ~ SPOKANE COUNTY HEALTH DISTRICT Inter-Office Comuni catlon Date January 31, 1977 TO Spokane County Planning Commission FROM Eugene C Prather, R S EIW SUBJECT ZE - 11 - 77 Two Family Residential to Residential Office The site is in an area where soils are usually suitable for use of on-site sewage disposal systems However, the site lies over the Spokane Aquifer which is currently under consideration by EPA as a "sole source" ps b R E c E I v E FE8 119T7 spoKAbE couaTrr PLAaNInG DEPARTMEaT SCHD-ADM-118 At said time and place any interested person may appear for, or agafnst, the granting of this application AGENDA, FEBRUARY 10, 1977 TELEPHONE 1V0. 456-2274 SPOKANE COUNTY PLANNING COMMISSION Time (PLEASE NOTE DATE) Thursday, February 10, 1977, 9 30 A.M Place Conference Room A, Court House Annex (Use Mallon Avenue entrance) 7.ONF C.HANCF_ Ki r"' t y ~ - - o U L~(21Q.5 3 7.F.-11-77 . PLANNING nEPA9 t PJ. 721 Jeffers nn Spokane, Wash. RE: Sunburn Arms 20 Unit Project N. Pines at Marietta Dear Steve: This letter and attached revised landscaping plan will confirm our conversation concerning landscaping the 4Jest portion of our property where we adjoin 3 duplex lots. I have taken thee as-built drawing and noted the area of concern which is the So. West corner of the property, We will put a minimum of 8 Pyramid Arborvitae and 14 Juniper Tams in a bark bed in the area designated. Steve, you have my assurance that this work will be satisfactorily completed on or before June 21, 1985. The contractor, Aspen Landscaping, has assured me he will have it done much before that date. The fence is up now. Steve, would you kindly contact Reed Ockey of the Building Inspectors Department and tell him to release our Certificate of Occupancy. Thank you for your excellent cooperation in this matter. Sincerely, l'► " Q~L'AND CO ~G y ~ lson GWN/sh Enc. S. 717 Pines Rd. • Spokane, Washington 99206 •(509) 928-2592
