2004, 05-06 Health District LetterMay 6, 2004 Greg Hobbs Catering and More 2801 N Park Spokane, WA 99212 Re: Catering and More, located ab2801 N Park, Spokane, WA Dear Mr. Hobbs: SPOKANE • REGIONAL H LT H DIS T R IC T 1101 West College Avenue Spokane, WA 99201-2095 509.324.1500 I TEL 509.324.14641 TDD www.SRHD.org As required under Section 1.05.145 of the Spokane Regional Health District Rules and Regulations for Food Service Establishments, this agency has reviewed the food menu, the food preparation sheet, and the building construction plans for the above -referenced project. We have found the plans to be complete that were received on March 5, April 2, and April 20, 2004, and are issuing our approval with the following conditions: 1) Operation is limited to no more than 12 events per year. 2) The establishment is required to connect to public sewer as soon as it is available. 3) If the existing septic system fails prior to connection to sewer, a new system will be required that meets current regulations. 4) Temperature logs are required for every event. Logs must include time and temperature for final cooking / reheating, departure, arrival, and holding of all potentially hazardous foods. We will be notifying the appropriate building department of our approval in order for you to obtain your building permits and commence construction. A fee of $60.00 per hour is charged for the plan review and pre -occupancy inspection. An invoice will be given to the person in charge at the time of the pre -occupancy inspection. The fee is to be paid within 30 days of receipt of invoice. A $65.00 late fee will be applicable if not paid within the 30 days. The current fee for a Caterer is $157.00. Application(s) are to be submitted at least fourteen (14) days prior to the projected date of opening to avoid paying a double permit fee. No applications(s) for permit(s) shall be accepted less than three (3) days prior to the date of opening due to the time needed to review the application. After you have completed construction, the Health District will conduct a pre -opening inspection. At the time of this inspection it will be verified that the construction is in compliance with the approved plans. (Any changes in menu or equipment shall be pre -approved by the Health District prior to construction.) Any equipment installed in the establishment is to be NSF approved or equivalent. Any home -style equipment found on the premises will have to be removed.
