PE-1206E Summerfield East 5th Addition
AIlan and I visi#ed this site on 3f22106 and offer the fallowing list of cancerns:
1. The swales at Queen and Calvi.n have not been comple€ed a.nd contain canstruction debris.
2. The swales were not carupleted afi E. 14611 and E. 14614 Queen.
3. Completed swales have sod but sad is higher than adlacent driveway causing ponding in
driveways.
4. E. 14+514 Queen has 2 cracked panels iu sFdewalk.
5. E. 14720 Queen has 1 cracked sidewalk panel and swales were not completed.
6. E. 14617 Queen has 2 Gracked panels in sidewalk.
7. The NIE corner of Queen and Calvin has crack ui laandicap ramp flare.
E. 14706 and E. 14712 each haVe 1 cracked panel in sidewal.kk.
4. E. 14720 has 2 cmked panels in sidewaik.
10. There are seaeral extra curb drops along Queen for driveways but none constructed. Nat sure why
fihey are there.
11. Al1 drywells and in.lets still have fahric in piace at the frame and grates.
The file Gantains a warranty bond in the amaunt of $31,951.90 dated 11126103 held by Safecv Insurance
Company of America. The band was far a period of Z years anai was to expire on 11126!05. John
Hohinan's e-mail dated 12i08105 required afinal wa.lkthrQUgh befare borad carz be reIeased. Safeco was
cantacted arid verified that this bund was renewed alnd vvill not expire m.zntil 11126106.
As far as the swaies are cQncemed, it appears tha# completian of them may be zesponsibility of the
individual property otivners or the developer. I'ni iiat sure E11eF has any control herc.
FZecoz-nmendation: Have Jahn ar Alysa conduct a wallcthrough with contractar tQ verify replacenlent of
cracked sidewalk panels and deternune necessary swale irriprovements. Have deveioper remave fabric
from drywells and inleis. C]nce this has been dar~e and carrections made, release bond and consider the
project closed.

protruding from face of curb at driveway approach.
4. N. 5124 Swale not completed and existing drywell may not function as intended.
5. N. 5214 Crack in curb and gutter.
6. Bums and Crown swale not completed on the southeast comer and cracked and chipped curb
and gutter.
Most driveway approaches have cracking in the gutter section, ranging from 1 or 2 to several that were
not individually noted on our review. Also it appears for the most part that 208 swales were not
constructed in accordance with the plans.
Recommendation - A walk-through (witli Eller?) conducted to determine items that should be corrected
under warranty.
Please let me know if you need more information.

I visited this site on 4/28/06 and found the following items of concern:
1. The swales along Wilbur Court and 7th in front of the residence located on the S/E comer are not
complete and the fabric is still in place on the drywells.
2. The swale located west of Wilbur Ct, along 8t' Ave., has not been completed and contains debri.
3. The drywell located in front of 704 S. Wilbur Ct. appears to still have fabric under the grate.
This project was constructed between December 2002 and June 2003 by Eller Corporation. The file
contains a Savings Assignment for Construction Performance in the amount of $149,800.00, issued by
Sterling Savings Bank, No. 39990383713. There is a letter from John Holunan, releasing this bond after
it was replaced with a warranty bond. This letter is dated 10/8/03. The warranty bond, in the file, is for

$29,000.00 and was issued by SaFeco Znsurance Campany nf America, dated 1016103. There is no
retease in the fale far the warranty bond. The warraniy period was ta expire ran 10f6l05. I cailed Safeca
and was told fihat the warranty bond vvas still active.
There are two sidewalk panels that have hairline cracks, but shauld not he a tripping hazard.
The fi1e cantains a Certifica#ion Package and Rec6rd Drawzngs.
RecQmmendation; Since the svaale completian is probably tied tQ the new hQanes that were built, natify
Eller to camplete the swale along 8th and remove fabric fram dryweils. Once this is daDe cansider this
project closed and write letter releasi.ng the warranty bond 46194417.

1. The swales along VViibur Cvurt and 74' in front of the residence located on the SIE
corner are nQt complete and the fabric is stili in place on the drywells.
2. The swale located west of Wilbur Ct, aPang $`h Ave., has not been cornpteted and
cantains detari.
3. The drywellloGated in front o£ 704 S. Wilbur Ct. appears to sti11have fabric urider
the grate.
This project was constructed hetween Decetnber 2002 and June 2003 by Ellet'
Corp❑ration. The fle contains a Savings Assignrnent far Constructaan PerfvrmaRr►ce in
the amount af $149,800.00, issued by Sterling SaVings Bank, No. 39990383713. There is
a Ieiter from Jvhn HQhman, releasing this bond after it was replaced with a warranty
bvnd. This letter is dated 10l8l03. The warranty band, in the file, is for $29,000.00 and
was issued by Safeco Insurance Company vf America, dated 1016l03. -f'here is na release
in the file fvr the warranty band. The warra.nty period was to expire on 1016105.
There are two sidewalk panels that have hairline cracks, but should nat he a tripping
hazard.
The file contains a Certif catian Paekage and Recvrd Drawings.
