Dear Rodney:
The purpose of this letter is to provide certification for the compaction of the subgrade for the roadbeds
in Shelley Lake 4th as well as the stormwater ponds. During construction the compaction tests were
not completed for the water and utility trench lifts or subgrade. However, the roads were core tested
after paving was complete in August of 2003 and passed compaction tests at that time. In addition,
during a site visit by John Saywers, PE and Gianna Adams, PE on March 19, 2007, the roads do not
t have any depressions or other indicators of poor compaction. Therefore, based on performance, we
will certify that the roads meet the requirements of the City of Spokane Valley. The ponds that were constructed on Carnine Lane have been reanalyzed using the 05-013 criteria that
was approved by the City for the stormwater ponds in the Shelley Lake projects. Based on this criteria,
the depth in the ponds was reduced from 0.67' to 0.5', thus removing the need for cation exchange
testing. The drywell was adjusted for this new depth and Pond B was cleared and an irrigation system
was installed. Pond A was surveyed at a 0.5' depth, so it was not adjusted.

It should be understood that the submittal provides certification for the initially constructed items associated
with this project. This certification only implies that the items shown were substantially built as designed, and
only valid the day the as-built survey and visual certification were performed by this engineer. This
certification makes no guarantee that other construction activities associated with overall plat build out such
as home builders or new home owners have not in fact dislodged, destroyed, filled in or in any manner
affected any of the items certified. It should be noted that this letter does not relieve the contractor or
developer from the warranties implied within their bonding documents.

Dear Buster:
On July 9th, 2004 a walk-through was completed for the above project with John Hohman
of the City of Spokane Valley Engineering Department. The following is a"puncfilist" of
items that need to be completed on Shelley Lake 4th Addition, per the approved
construction plans:
1. The W.S.D.O.T. Catch Basin located at the northwest curve of Shelley Lake Lane
has an approximate 5" depression. The plans call* out for a 2" depression. The
catch basin will need to be raised with at least a 2" riser. At this time, the '
, concrete apron around the catch basin will need to be replaced as well.
2. There are many cracks in the curb/gutter that will need to be removed and
replaced. Probably due to constrction around the site, the tops of the curbs have
been run over and have fractured. I have marked the curbs needing replacement
with a red "mazker" crayon. The marks will not last very long, so I would advise
Bob Heitman or Shawn Montez to have a look at the curbs marked with a"Red"
circle. It was determined by the either the Spokane Valley or myself which curbs
will need repairs. .
3. Pond A and Pond B are adequately sized per the design plans. However, the
ponds will more than likely need to have some sort of irrigation means to aid in
the germination of the dryland se.ed. The ponds currently are filled with weeds and are not consistently "flat" at the bottom of the pond. The pond should be
graded to create a flat bottom throughout the pond.
4. Pond A will need a new concrete apron at the inlet. The current inlet is cracked
which seems to be from erosion of material underneath the apron.
5. Due to the new location of Pond C, the City of Spokane Valley has informed me
- that they still do not have the most current set of plans. I contacted Autumn
Friday @ CLC Associates to find out what the status of those plans are and when I
can get a copy. I left a message for her to call me back. Once I have received
+ those plans I can do a more thourough inspection of the project and make sure that
Pond C and D are adequately sized and in conformance with the design plans.

The top soil placed in the ponds on this project (and Shelley Lake 2"d Addition) was sampled and
tested for Organic Content on March 31. At the time of my testing, the ponds had already been
hydroseeded. A minimum of 2% organics is required; our testing indicates 1.1 % organics
present.
Since the ponds 6ad been seeded, I obtained an additional sample and submitted it to an
agricultural soils lab for the alternate Cation Exchange Capacity test. I received results this
morning indicating 9.60 Meq/100g compared to the requirement of at least 15 Meq/100g.
The soil in the ponds will need to be replaced with soils acceptable to the City which meet the
County stormwater requirements. I strongly urge you to have us test the soil prior to placement.

Thank you for sending Sean Montez to meet with us on March 15, 2004 regarding the -
construction deficiencies ai the Shelley Lake development. The construction issues are
related to four phases of the project. Based upon owr site visit, we have compited the
following list of deficiencies that must be addressed. Please check with your inspector,
STI Northwest for any additional items that they may have.
- Shellev Lake PUD
• Replace concrete cap at 4`h aad Cannery with ACP.
• Fine grade three ponds located near 1Vloore Lane per design dimensions, irrigate,
and establish sod.
• Gravel must be removed in the swale bottoms at the north end of Sonora Lane and
replace witb sod. .
Shellev Lake 2"d Addition
• Fine grade swales per design dimcnsions, irrigate, and establish sod. Ensure that
all dryweils are 8 inches above swale borioms. '
• Cation exchange testing must be complcted.
• Repair asphalt and curb settlement on Shelley Lake Lane in the vicinity of sewer
manhole.
• Repair concrete cross gutter at Galway and 9th. Mortar lid franae to sewer manhole
' cone.
• Replace curbs at island entry fealure.
• Rotchford: repair two settlement areas of street, curb, sidewalk• one near

Dear Mr. Hohman: At the request of our client, Shelley Lake Development, we wish to request a reducrion of established road
construction bonds to a level which is proportional to uncompleted work associated with this project.
Having reviewed the construction to this point and the bond estimate prepared by GMM for Spokane County and
dated March 24, 2003, we believe the specified road work is approximately 95 percent complete. Our certification
of the storm drainage system is withheld pending a redesign to be submitted by CLC Associates.
We estimate the value to complete the final work on this project to be approximately $34,000 including all
currently specified storm drainage work. We hereby request a reduction of established road construction bonds
which are proportional to uncompleted work associated with this project.
It should be understood that tliis certification only implies that the items shown on the Record Drawings were
substantially built as designed, and only valid at the time the as-built information and visual certification were
performed by this firm. It should be noted that this letter does not relieve the contractor or developer from the
warranty periods and contractual provisions expressed or implied within their bonding documents.

Before requesting a re-TV on the sewer, the lift holes in MH 409 need to be mortared.
Before submittal of the sewer as-builts and acceptance by the County, a 14' wide X 6" deep
compacted gravel road will need to be built from the end of the pavement on Carnine to MH
411. Also a locking ring and cover needs to be installed on MH 411 (see Standard Drawing
U-14).
Please let me know when this work has been completed so I can arrange for the re-T1/ and
bond reduction.

Dear Buster:
Per your request, I have reviewed the work completed to date on this project and have
encountered two serious problems.
STI Northwest, Inc. was not contacted for density testing on the utility trenches, water lines or
road subgrade. Nor were we made aware that gravel was being placed. We cannot verify or
certify that the subgrade and trench compaction is within the County/City of Spokane Valley
specifications. We did perform density testing on the gravel top course, but cannot verify or
certify the gravel depth or if it was placed in layers as required by specification.
The drainage system as constructed does not match the drainage as shown on the approved plans.
Specifically, the pond shown in Tract 'C' was not constructed and a pond was added outside of
the tract to the south east. Ancillary piping and an a.dditional catch basin were also added. Prior
to acceptance of this project, your design engineer will need to submit the revised drainage plans
to the County and/or City for acceptance. The WSDOT catch basin at 23+25 on Shelley Lake
Lane is depressed in excess of three inches, but this may also be part of the drainage re-design.
Additionally, a concrete splash pad was not constructed in Tract 'B' as required. Several
borderline curb cracks were noted, but those around 23+75 RT on Shelley Lake Lane; 14+50 RT
on 9th Lane; 16+50 RT on 9th Lane and 20+50 RT on 9`h Lane need to be repaired. About ten
feet of damaged curb at 19+10 RT on 9`h Lane will need to be replaced.
All ponds in tracts must be irrigated and sodded. I did not verify any dimensions pending the
approval of the re-design.
Street signs were not installed.

On 7/14 I was called by Susan Murphy concerning problems with the elevations of two manhole
rims on Shelley Lake 4`h. This conversation raised questions in my mind since the manholes had
been installed some time ago and the question of rim elevations would not be raised again until
after gravel was placed.
I called Sean at C&B and was told he'd justlearned that Inland had prepared the subgrade at
Shelley Lake and placed gravel. We had not been made aware of any of this activity. I called
Buster and was told to call Guy Anderson at Inland. Guy told me he was a subcontractor and it
was not within his scope to be notifying us of construction activities. Guy also told me the
placement of the gravel had been completed and checked by Inland's QC.
I visited the site on 7/15 and found that the placement of the gravel had indeed been completed.
I instructed Mike Cunningham to back scatter the top of the gravel to check for compaction and
also to test at a depth of 4" to try to determine compaction of the first lift (if indeed the gravel
was placed in two lifts).
We did not check the subgrade, water or utility crossings for compaction.

During the course of construction, this MH did not end up quite at the planned
location. An eccentric cone was added and turned so that the MH access would
be behind the curb line. This MH is quite deep (-25') and it will be necessary to
replace all of the steps such that they're above the shelf, not the upstream pipe
as required because of the turned cone.
Questions: 1) Any problem with moving the steps?
2) Since the lid will be behind, but adjacent to the curb, do we need to pave
around it?

of ineasuring the depth of aggregate base and to probe the subgrade. Eleven site were picked at random

Field Observa6ons
Arrived on site to observe laying of one lift of asphalt. Upon arrival, the tack coat had been applied to all
curbs and existing vertical pavement. Existing asphalt had been freshly saw cut. Paving crew began
laying asphalt at south end of Shelley Lake Lane. The asphalt wa.s laid according to design specifications,
which called for a 2 inch lift. Sample of asphalt was collect@d on Shelley Lake Lane at Stn. 19+10. Field
temperature of approximately 275° F met specifications, along ivith a subgrade temperature of 6i9°.
Using a maximum density of 151.4 lb/ft3, the asphalt density ranged from 88% to 96%. Since not all thE
asphalt met density specifications of at least 92%, crews rerolled problem areas. Upon rErolling, retests of
asphalt met or exceeded minimum specifications.

Contractor was adjusting manhole rim etevations and grouting at time of arrival. CSTC had been
backfilled around completed manholes but was not going to coaduct any compactioa activities unril the
grout has set up. Two completed manholes were tested by soil probe. R,esults showed that CSTC had
been placed around ihe manhole with no water added. Contractor said thax the manholes would 6e ready
to test later today. STI will return to tesst compaction of CSTC around manholes at a later date.

Arrived on site to inspect aad test compaction of CSTC.
Upon arrival at the time specified by the contractar, both lifts for a total of 6" of CSTC were placed,
graded, and compactcd and Inland Asphalt had ieft the site. Re.suits of Inland Asphalt's density tests
camied to one teirth of one percerrt by nuclear densometer of the top lift were painted on the gravel
surface. -
The upper lift of CSTC was tested for compadion by nuclear densometer in backscatter mode and the
lower lift was tested by driving a pin through the CSTC and testing at 4" by nuclear densometer.
Approximate maximum density value was obtained by comparing an Inland test shot to my test shot at
the same locaiion and calculating backwards. A sample of the CSTC was obtained for lab test of
maKimum density per ASTM D-1557. All test locations but four of fifty-seven met or exceeded Spokane
County staadards of 95% compaction based on the approximate maximum density value obtained from
Inlaad's resuhs. Those four were 94% of maximum.

Curb reveals appeared to be correct for 2" of class "B" asphalt concrete pavement per detail "B" on page 6
of plaas. Contractor was adjusting manhole rim elevations at time of testing. ST'I will retum to test
compaction of CSTC around manholes at a later date.

Amived on site to inspect aad test concrete for cross drain on 9`h at Galway Lane.
Cross drain was installed per detail sheet supplied by CLC Associazes. .
A manhole and waier valve were in the cross drain area, the water valve was raised to m,aich the cross
drain surface. The manhole was -1 inch too high to match the cross drain, but could not be altered. The
ooncrete was tapered as smoothly as possible to the manhole rim. Another water valve was in the north
curb -lft. northwest of the flow line. See detail drawing for locations of manhole and water valves.
The asphatt pavment was sawed aad removed per detail sheet aad the underlying gravel was removed to
match the depth of the existing concrete curbs. 'Ihe disturbed gravel was watered and consolidated with
the Wacker WP-1550. The flow line was formed as shown in the detail drawing
Concrete: 76° F, 5.5" slump, 8.0 gallons water added, 4.2% entrained air, one set 6" by 12" test cylinders
cast.

Field Observations
Arrived on site to test concrete for cwb and gutter, type R, by plan specifications.
At the time of arrival, the conuactor had laid concrete up to the north side of 9" Lane around 19+00.
Checked string level and curb tayout. The level and layout appeared to follow the specifications and grade
as shown on plan drawings. The curb machine was broken for an undetermined period of time.
Returned at 1:15pm to test concrete for curb and gutter, tYPe R, bY Plan specifications. Contractor had 2
mixers running and the oub machine was working again. Gathered sample at southwest coraer of gP and
Shelley Lake Lane.
Cancrete: 77° F, 0.75" slwmp, 84° F ambieat temperature, 4.8% entrained air, cast one set 6" by 12" test
cylinders
Curb inlets and grate inletss were placed as shown on plans.

Arrived at jobsite at contractor's request to observe installation of four (4) drywells.
Type "B" double barrel drywells were placed in three of the locations (Tract A@ 22' right of STA 2313,
Tract B@ 22' left of STA 1170, & Tract D@ 22' left of STA 1213), and a type "A" single barrel drywell
was placed at the north side of Tract A@ 22' left of STA 1213. Bottom of type "B" drywells is
approximately 11.5 ft. below existing ground level and type "A" is at 7.5 ft. Native material consists
coarse clean sand with decomposed bedrack. The excavations measure approximately 17' x 18' at the top.
They are lined with geotextile filter fabric. The barre] of the drywell was not wrapped in accordance with
updated Spokane County Standard Plan B-lA. Excavation measurements and conh-actor's count of
buckets indicate 4045 cu. yds. of 2.5" drain rock were placed in the excavations for the double barrel
drywells. The filter fabric was brought over the drain rock to the drywell cone and a small piece of filter fabric was
placed over the cone. T'he top of excavations were backfilled with native material to existing grade and a
steel plate was placed over the opening.

408 to MH 407 at 2'-3' below ga,de. .
Due to the extremely rocky nature of the native "Valley Gravel" being used for backfill, compaction
testing by nuclear densometer was not feasable. The backfill at the above raentioned areas appeared to be
firm and unyielding as tested by t-handle soil probe.
A"Jumping Jack" was being used to further compact the backfill against Manholes #408, 407. Sewer
service was being placed Block 21ot 3 and 2. All of the area tested appeared as per plans• -

Observed compaction of subgrade over sanitary sewer line on the proposed Cannine Lane from MH 411
to MH 443 at - grade, MH 403 at -lft. to MH 412 at - 8ft. below grade, and on the pmposed 9'hLane
from MH 404 to MH 405 to sta 22+00, just west of MH 406 at --4ft below grade.
Due to the extremely rocky nature of the native "Valley Gravel" being used for backfill, compaction
testing by nuclear densometer was not feasable. 'Ihe backfiU at the above mentioaed areas appeared to be
fum and unyielding as tested by t-handle soil probe with the exception of an area on the right side of
Carnine Lane between sta. 1+65 aad 2+20. Soil probe penetration was -12" in this area, indicating that
compaction of the last lift in this area had been missed.
I pointed this location out to the site supervisor, it was recompacted while I was still on site, and then
appeared to be firm and wayielding by soil probe test.
A"Jumping Jack" was being used to further compact the backfill against Manholes #411, 403, and 412.

Field Observations Arrived at jobsite for onsight inspection. Observed contractor installing 8" PVC main sewer line between
stations 18+50 and 19+00 on 9h lane. Also observed placement of MH ai station 18+50. Pipe
connections aad MH sections appeared to be installed properly with gaskets and lubricant. 4" Side
services were stubbed off 8" mainline at this time. Sewer mainline was being properly bedded with
bedding saad. Contractor checked elevations on MH mentoined above, before and after setting MH and
inserting sewer mainline. Contractor backfilled first lift over bedding as being installed. Cantractor had
not yet compacted.
