Dear Mr. Hohman:
At the request of our client, Shelley Lake Development, we wish to request a reduction of established road
construction bonds to a level which is proportional to uncompleted work associated with this project.
Having reviewed the constxuction to this point and the bond estimate prepared by GMM for Spokane County and
dated March 24, 2003, we believe the specified road work is approximately 95 percent complete. Our certification
of the storm drainage system is withheld pending a redesign to be submitted by CLC Associates.
We estimate the value to complete the final work on this project to be approximately $34,000 including all
currently specified storm drainage work. We hereby request a reduction of established road construction bonds
which are proportional to uncompleted work associated with this project.
It should be understood that this certification only implies that the items shown on the Record Drawings were
substantially built as designed, and only valid at the time the as-built information and visual certification were
performed by this firm. It should be noted that this letter does not relieve the contractor or developer from the
warranty periods and contractual provisions expressed or implied within their bonding documents.

During the course of construction, this MH did not end up quite at the planned
location. An eccentric cone was added and turned so that the MH access would
be behind the curb line. This MH is quite deep (-25') and it will be necessary to
replace all of the steps such that they're above the shelf, not the upstream pipe
as required because of the turned cone.
Questions:
1) Any problem with moving the steps?
2) Since the lid will be behind, but adjacent to the curb, do we need to pave

Field Observations
Arrived at jobsite for onsight inspection. Observed contractor installing 8" PVC main sewer line between
statioos 18+50 and 19+00 on 9'h lane. Also observed placement of MH at station 18+50. Pipe
connections and MH sections appeared to be installed properly with gaskets and lubricant. 4" Side
services were stubbed off 8" mainline at this time. Sewer mainline was being properly bedded with
bedding sand. Contractor checked elevations on MH mentoined above, before and after setting MH and
inserting sewer mainline. Contractor backfilled first lift over bedding as being installed. Contractor had
not yet compacted.

to MH 403 at 4ft. below grade, and from MH 403 at -6ft. to MH 412 at - 12ft. below grade.
Due to the eartremely rocky nature of the native "Valley Gravel" being used for backfill, compaction
testing by nuclear densometer was nat feasable. The backfill at the above mentioned areas appeared to be
firm and unyielding as tested by t-handle soil probe.
After conversation with the site representative, Shawn, he stated that he would lightly water the next lift
as it is Ueing pla.ced.

-12" were added since inspection earlier today.
Due to the extremely rocky n,ature of the native "Va11ey Gaavel" being used for backfill, compaction
testing by nuclear densometer was nat feasable. The backfill at the above mentioned areas appeared to be
firm and unyielding as tested by t-handle soil probe.
The site representative determined that no water addition was necessary, only the surface of the stockpiles
aPPeared drY.
The contractor was conducting preliminary air testing of the sewer line between the above mentioned
ma.nholes.

to MH 403 at - grade, MH 403 at -lft. to MH 412 at - 8ft. below grade, and on the proposed 9" Lane
from MH 404 to MH 405 to sta 22+00, just west of MH 406 at -4ft below gra.de.
Due to the extremely rocky nature of the native "Valley Gravel" being used for backfill, compaction
testing by nuclear densometer was not feasable. The backfill at the above mentioned aras appeared to be
firm and unyielding as tested by t-handle soil probe with the exception of an azea on the right side of
Carnine Lane between sta. 1+65 and 2+20. Soil probe penetration was -12" in this area, indicating that
compa.ction of the last lift in this area had been missed.
I poiated this location out to the site supervisor, it was recompacted while I was still oa site, aad then
appeared to be firm and unyielding by soil probe test.
A"Jumping Jack" was being used to finther compact the backfill against Manholes #411, 403, and 412.

Arrived at jobsite at contractor's request for air pressure testing of new sewer lines. All air pressure tests
of sewer liaes between manholes on Shelly Lake Lane, Carnine Lane, and Ninth Lane met or exceeded
Spokane County Standards.
The cone on Manhole #24 aoes noi line up properly with the steps in the barrel of the manhole.
Manhole #411 does not have any inlets cast in the base for future development.

Field Observations '
Arrived at jobsite at contractor's request to observe installation of four (4) drywells.
Type "B" double barrel drywells were placed in three of the locations (Tract A@ 22' right of STA 2313,
Tract B@ 22' left of STA 1170, & Tract D@ 22' left of STA 1213), and a type "A" single barrel drywell
was placed at the north side of Tract A@ 22' left of STA 1213. Bottom of type "B" drywells is
approximately 11.5 ft. below existing ground level and type "A" is at 7.5 ft. Native material consists
coarse clean sand with decomposed bedrock. The excavations measure approximately 17' x 18' at the top.
They are lined with geotextile filter fabric. The barrel of the drywell was not wrapped in accordance with
updated Spokane County Standard Plan B-lA. Excavarion measurements and contractor's count of
buckets indicate 4045 cu. yds. of 2.5" drain rock were placed in the excavations for the double barrel
drywells.
The filter fabric was brought over the drain rock to the drywell cone and a small piece of filter fabric was
placed over the cone. The top of excavations were backfilled with native material to existing grade and a
steel plate was placed over the opening.

Field Observations Arrived on site at the contractors request to test subgrade for curb and gutter.
At the time of arrival, the contractor had compacted subgrade for curb and gutter on the left and right
sides of 9h Lane and Lane'A'.
An STI proctor value of 137.1 pcf, was used for density testing.
All tests produced results of 92% or better.
The contractor had a loader and two dump trucks on site, and was moving excess fill off site.

Arrived on site to test concrete for curb aad gutter, type R, by plan specifications.
At the time of arrival, the contra.ctor had laid concrete up to the north side of 9`h Laae arouad 19+00.
Checked string level and curb layout. The level and layaut appeared to follow the specifications aad grade
as shown on plan drawiags. The curb machine was broken for an undetermined period of time.
Returned at 1:15pm Lo test concrete for curb and gutter, type R, by plan specificaiions. Contra.ctor had 2
mixers running an.d the curb machine was working again. Gathered sample at southwest corner of 9~hand
Shelley Lake Lane.
Concrete: 77° F, 0.75" slump, 84° F ambient temperature, 4.8% entrained air, cast one set 6" by 12" test
cylinders
Curb inlets and grate inlets were placed as shown on plans.

Arrived on site to insped and test concre#e for cr+oss drain on 9`h at Galway Lane.
Cross drain was installed per deta,il sheex supplie,d by CLC Associates. .
A manhole and waxer valve were in ihe cross drain area, the water valve was raised to match the cross
. drain surface. The manhole was -1 inch too high to mazch the cross drain, but could not be altered. The
concrete was tapered as smoothly as possible to the manhole rim. Another water valve was in the north
curb -lft. northwest of the flow line. See detail drawing for locations of manhole and water valves.
The asphatt pavment was sawed and removed per detail sheet and the underlyiug gravel was removed to
m,atch the depth of die existing concrete aurbs. The disturbed gravel was watered aad consolidated with
the Wacker WP-1550. The flow line was formed as shown in the de#ail drawing
Concrete: 76° F, 5.5" slump, 8.0 gallons waxer added, 4.2% entrained air, one set 6" by 12" test cylinders
cast.

Field Observations
Arrived on site to inspect and test compaction of CSTC.
Upon arrival at the time specified by the contractor, both lifts for a total of 6" of CSTC were placed,
gra,ded, and compacted and Inland Asphalt had left the site. Results of Inland Asphalt's density tests
carried to one tenth of one perceat by nuclear densometer of the top lift were painted on the gravel
surface.
The upper lift of CSTC was tested for compaction by nucle,ar densometer in backscatter mode and the
lower lift was tested by drivin,g a pin through the CSTC and testing at 4" by nuclear densometer.
Approximate maximum density value was obtained by comgaring an Inland test shot to my test shot at
the same location and calculating backwa.rds. A sample of the CSTC was obtained for lab test of
maximum density ger ASTM D-1557. All test locations but four of fifty-seven met or exceeded Spokane
County standards of 95% compaction based on the approximate maximum density value obtained from
Inland's resuhs. Those four were 94% of maximum.

Curb reveals appeared to be conect for 2" of class "B" asphalt concrete pavement per detail "B" on page 6
of plans. Contractor was adjusting manhole rim elevations at time of testing. STI will return to test
compaction of CSTC around ma.nholes at a later date.

Contractor was adjusting manhole rim elevations and grouting at time of amival. CSTC had been
backfilled around completed manholes but was not going to conduct any compaction activities until the
grout has set up. Two completed manholes were tested by soil probe. Resuhs showed that CSTC had
been placed around the manhole with no water added. Contractor said that the manhole,s would be r+eady
to test later today. STI will retuum to test compaction of CSTC around manholes at a later date.

_ Arrived on site to observe laying of one lift of asphalt. Upon arrival, the tack coat had been applied to all
curbs and existing vertical pavement. Existing asphalt had been freshly saw cut. Paving crew began
- laying asphalt at soutll end of Shelley Lake Lane. The asphalt was laid according to design specifications,
which called for a 2 inch lift. Sample of asphalt was collected on Shelley Lake Lane at Stn. 19+10. Field
temperaturE of approximately 275° F met specificarions, along with a subgradc temperature of 69°.
Using a maximum density of 151.41b/ft3, the asphalt densit_y ranged from 88% to 96%. Since not all the
asphalt met density specifications of at least 92%, crews rerolled problem areas. Upon rerolling, retests of
asphalt mct or exceeded minimum specifications.

The following lines were cleaned by blowing back material from uphill and vacuuming; MH 23
(164.3/8.9) to NIH 403 (164.0/9.3) and MH 410 (165.9/9.8) to MH 24 (164.9/8.8) including
intermediaries. Contractor manually removed some visible debris from the bottom of MH 23, 408, and
409.

for the evaluation. All site examined showed the subgrade to be firm and unyielding when probed. See the

RECORD DRAWflJGS & ENGINEER'S STATEMENT
1nspection Re4uirements
These requirements apply #o all pubfic and private sev+rer lines eight inches and larger in
diameter, and to sewer service stubs extended from those lines to the property lines of individual
parcels. Note that all sewer service lines six inches and smatler in diameter, ather than those
service stubs noted above, wili be inspecteti by SRokane County Division of Utilities.
On ali sevrrer constructlon for development, inspections wfil be performed by the Sponsor's
Engineer or his/her designated representative. At the request of the sponsor. the County may
provide ihe sewer inspection services, provided that ihe sponsor agrees to pay ali costs
associated with the County inspections and provided that the County detemnines that staff is
available to perform the inspecttons.
The inspection of sewers shaU include the items listed beiow. However, the listing pravided
below is not intended to be all inclusive. It will be the responsibility of the Sponsor's Engineer to
determine add;tional inspection activities that may be needed for a specific project in order to
rsport substantial conformance of the project with the County's standards.
1. Conformance of alf construction materials wlth County standards shall be verified prior to
instatlation.
2. The sewer trench shall be inspected for proper dimensions and pipe zone clearances priar to
placement of pipe.
~3. The placement and compaction of the pipe zone materlal and bedding shall be inspected.
4. The pipe joints shall be inspected visuafly for proper insertion.
5. Norizontal alignment and grade of the pipe shatl be checked for canformance to the standards
prior to backfilfing of the trench.
6. Sewer service tees and stubs shall be inspected for correct installation prior !o backfilling of
the trench.
Accurate measurements shall be made and recorded to faciiitate the reeslablishment o€
sewer service tee locations and stub end locations. The placement of the required stub
markers shall be verified.
8. Manholes shall be inspected for proper materials, locatian, assembty, and installation.
9. Trench backfilling operations shall be observed and compaction tests shall be performed.
Minimum requirements for backf+lling shall be as set forth in the acxepted pians and/or
specifications for the project.
10. Mandrel testing, hydrostatic and/or air pressure testing, and manhole testing shall be
performed following completion of trench backfifling operations. The testirtg shall be observed
until passing resulis are obtained. Manhole testing shall be performed on ten percent of the
manholes in the project, in accordance with Department of Ecology's "Criteria for Sewage
Works Design".

11. The Sponsor's Engineer shall schedu{e teievision inspection of the sewer lines through the
Division of Utilities once the other testing has been satisfactority campleted. The Division of
Utilities wiU then perform the televlsion inspection and review the tapes for deficiencies
requiring correction or repair,

12. Daily inspection reports shall be pcepered, summarizing consiruction activities, contractor
work force and work perioti, testing results, problems encountered, and oiher pertinent
information.
The project sponsor or the sponsar's Construction Engineer shall notify the Division of Utilities 48
hours in acivance of the siart of construction of ttle sewer system, and shall provide a generalized
schedule for the progress of the work. Couniy inspection personnel will tnake occasional site
visits to inspect the wark and do materials testing as deemed necessary. County inspection
personnel wiit address concerns and questions to the Engineer of Record. The vost of these
occasFOnal site visits wiil be charged to the Corlstructian Permit for the work.
The sponsor will beer the cost of sll inspections and materials tesfing. The Division of Utilifies
shall have access to a!I construction jnspection records and reports. Before the County accepis
the sewer system, the Sponsor's Engineer shall submit all construction records to the Division of
Utilities. This shall include but not be limited to Daily Inspection Reports, cvrrespondence,
manufacturers' certifications, and material test reports.
When changes to the design are necessary, the sponsor shall be responsible for coordinating the
proposed design changes with the sponsor's Design Engineer. The sponsor's Oesign Engineer
shall fon+vard two copies of the proposed plan changes, together with related catcutations, to the
Division of Utilities for review and acceptance prior to construction.
The Sponsor's Engineer shall schedule a final inspection meeting between the Sponsor's
Enginesr, the sponsor, and the Oivision of Utilities at the completion of construction and testing of
the sewer system. Deficiencies found shall be corcected by the sponsor prwr to the County's
acceptance of the sewer.
Record Orawinqs 8 En4ineer's Statement
During oonstruction the sponsor's Surveyor andlor Construction Engineer shall record any
changes to the accepted plans. Alt substantive changes shall be indicated and noted on the
accepted final sewer pfans and the plans shall be annocated as "Record Drawings". In those
cases where the County provides the inspection services, the County will prvduce the Record
Drawings.
Side sewer . stub sketches shall also be prepered on the County's standard form. As-buitt
elevations, locations, pipe lengths, and pipe slopes must be verified on the Record Drawings. The
faltawing statement shall be lettered on fhe Record Drawings and stamped and signed by the
sponsor's Construction Engineer on the Record Drrawings: "1 have reviewred the construction of
this sewer system and to my knawledge find it to be ln substantiai conformance with the accepted
plans and Spokane County Standards except as noted." ,

PROTOCOL FOR TELEYlSIUN INSPECTION OF SEWERS
In the interest of developing a consistent methodology for the coordination and
review of television inspections, the fo!lowing steps have been established:
1. PRE-PAVING TELEViS14N INSPECTION
a) At teast two weeks prior to the desired pre-paving television inspection, the Conttactor requests
the television inspection through the Sponsor's Engineer. The Engineer submits the requesi (on
standard form) to the Division of Utilities Sewer Design Sectian. (Utflity contacts ere B. Urhausen
and G. Repp).
bj Sewer Design Section schedutes tetevision inspection with the Wastewater Operations Section.
c) In order for the inspection to take place, sewer mains wfll. ne8d to be high-pressure cleaned,
mandret tested, and pressure tested. In addition, manholes will need to be accessible by the N
van. 8ackfiil must be in place around the cone sections of the manholes. If the contractor has
placed temporary steet plates on top of the manholes, then the contractor wiii need to provide
manpower and equipment to remove and replace the plates to facllitate the television inspection.
dj Detailed television inspection is undertaken, noting tee IocaUons/stations, irregularities, defects,
etc.
e) The Contractor shall pay the cost of the pre-paving tEtevision inspection.
2. REVIEW OF PRE•PAVING TELEVISION INSPECTION
a) When the Sewer Design Section receives the VHS tapes and fault log from the Operations
Section, the fauit log will be reviewed for nated defects. The fapes and !og will tfien be forwarded
to Engineer for review.
b) Engineer reviews tapes and fog, and makes determinatian of any repairs fhat nesd to be
undertaken.
c) Engineer coordinates with Sewer Design Section Manager for concurrence on repairs ihat are
_ needed. In some cases, this may necessitate joint review of the tapes.
d) After concurrence between the Engineer and the Sewer Design Section Manager, the Engineer
will stipulate the required repairs to the Contractor.
3. ACCEPTANCE STANDARDS FOR GRADE DEVIATIONS
a) The Standard Specificstions stipulate that reverse grades are not acceptable. In the review of
tetevision inspection tapes, the Engineer and the Sewer Design Section Manager wiN consider
the combined effects of p{pe joints, allowed variations in pipe diameter, and grade on any
entrapped water appearing on the inspection tapes.
4. POST-PAVING TELEVIStON INSPECTION
a) After the roadway is substantialty complete above the sewer, a post-paving television inspeetion
will be scheduled, in the same manner as ihe pre-peving television inspection.
b) This television inspection will be less deiafied than the first, and will be undertaken to check for
pipe cleanliness, and to identify any structurel defects that may have occurredlpresented since
the initial tefevision inspection.
c) The review of the tapes and concurrence on needed repairs will proceed in the same fashion as
identified for the pre-paving television inspection.

d) The cos# of the first past-paving television inspection shafl not be charged to the Contractor. The
cost of any subsequent television inspections performed to confirnn the adequacy of repairs will
be charged to the Contractor.
5. REMEDY PROVlSIONS -
a) in the case of a pipeline swate, or other defect, that is identified during the post-peving television
inspection, the Engineer and the Sewer Design Section Manager shatl determine the need for
repair work to correct the defect.
b) In certain cases, in lieu of undertaking the repair, the Contractor may be required tn pay the
estfmated cost of making the repair to the County. Such payment will tfien be deposited in the
County's sewer ►epair and replecement fund. '
6. MANHOLE IIdSPECTIONS
a) The Engineer shatl inspect the sewer manholes for cortformance writh Spokane County
Standards. Observations regarding the conditfon of manholes made by the television inspection
crew are to be considered as supplementary information. and are not intended to replace the
Engineer's inspectfon.
7. MAKING CONNECTIONS TO NEW SEWER LINES
No connections to the new sewer lines will be allowed until the County has accepted the sewer lines for
oaeration and marntenance. 8efore acceptance can occur, the Engineer must provide acceptable record
drawings for the project to tfie Divis+on of Utilfties, together with the required Engineer's Siatement.
Additionally, the wamanty period security must be provided by the developsr as stipulated in the
development agreement.
