It should be understood that this submittal provides certification for the initially constructed items associated with
this project. This certification only implies that the items shown were substantially built as designed, and only
valid the day the as-built survey and visual certification were performed by this engineer. This certification makes
no guarantee that other construction activities associated with overall plat build out such as home builders or new
home owners have not in fact dislodged, destroyed, filled in or in any manner affected any of the items certified. It
should be noted that this letter does not relieve the contractor or developer from the warranties implied within their
bonding documents.

During the course of construction, this MH did not end up quite at the planned
location. An eccentric cone was added and turned so that the MH access would
be behind the curb line. This MH is quite deep (-25') and it will be necessary to
replace all of the steps such that they're above the shelf, not the upstream pipe
as required because of the turned cone.
Questions:
1) Any problem with moving the steps?
2) Since the lid will be behind, but adjacent to the curb, do we need to pave
around it? -

Arrived at jobsite for onsight inspection. Observed contractor installing 8" PVC main sewer line between
stations 18+50 and 19+00 0n 9* lane. Also observed placement of MH at station 18+50. Pipe
connections and MH sections appeared to be installed properly with gaskets and lubricant. 4" Side
servic,es were stubbed off 8" mainline at tlus time. Se°wer mainline was being properly bedded with
bedding sand. Contractor checked elevations on MH mernoined above, before and after setting MH and
inserting sewer m,ainline. Contractor backfilled first lift over bedding as being installed. ContYactor had
not yet compacted.

Obsecved compadion of subgrad,e over sanitary sewer line on the proposed Carnine Lane from MH 411
to MH 403 at -4ft. below grade, and from MH 403 at -4ft. to MH 412 at - 9ft. below grade. Two lifts of
-12" were added since inspedion earlier today.
Due to the eactremely rocky nature of the native "Valley Gravel" heing used for backfill, compaction
testing by nucleaz densometer was not feasable. The backfill at 1he above mentione,d areas appeared to be
firm and unyielding as tested by t-handle soil probe.
The site representaxive determined that no water addition was necessary, only the surface of the stocicpiles
aPPeared drY•
The contractor was canducting preliminary air testing of the sewer line between the above mentioned
manholes.

Field Observations
Arrived at jobsite at contractor's request to observe installarion of four (4) drywells.
Type "B" double barrel drywells were placed in three of the locations (Tract A@ 22' right of STA 2313,
Tract B@ 22' left of STA 1170, & Tract D@ 22' left of STA 1213), and a type "A" single barrel drywell
was placed at the north side of Tract A@ 22' left of STA 1213. Bottom of type "B" drywells is
- approximately 11.5 ft. below existing ground level and type "A" is at 7.5 ft. Native material consists
coarse clean sand with decomposed bedrock. The excavations measure approximately 17' x 18' at the top.
They are lined with geotextile filter fabric. The barrel of the drywell was not wrapped in accordance with
updated Spokane County Standard Plan B-lA. Excavation measurements and contractor's count of
buckets indicate 4045 cu. yds. of 2.5" drain rock were placed in the excavations for the double barrel
drywells.
The filter fabric was brought over the drain rock to the drywell cone and a small piece of filter fabric was
placed over the cone. The top of excavations were backfilled with native material to existing grade and a
steel plate was placed over the opening.

An STI proctor value of 137.1 pcf, was used for density testing.
All tests produced results of 92% or better.
The contractor had a loader and two dump trucks on site, and was moving excess fill off site.

At the time of arrival, the eontractor had laid concrete up to ihe north side of 9`hLane around 19+00.
Checked string Ievel and curb tayout. The level and layout appeared to follow the specifications and grade
as shown on plan drawings. The curb machine was broken for an undetermined period of time.
Rehuned at 1:15pm to test concrete for curb and gutter, type R, by plan specifications. Contractor had 2
mixers nwning aad the curb machine was working again. Gathered sample at sauthwest comer of 9`~ and
Shelley Lake Lane.
Concrete: 77° F, 0.75" slump, 84° F ambieIIt temperature, 4.8% entrained air, cast one set 6" by 12" test
cylinders

Amived on site to inspect aud test compaction of CSTC.
Upon arrival ai the time specified by the comractor, both lifts for a total of 6" of CSTC were placed,
graded, and compacted and Inland Asphah had left the site. Results of Inland Asphalt's density tests
camied to one tenth of one perceni by nuclear densometer of the top lift were painted on the gravel
surface.
The upper lift of CSTC was tested for compaction by nuclear densometer in backscatter mode and the
lower lift was tested by driving a pin through the CSTC and t,esting at 4" by nuclear densometer.
Approximate n~~ximum density value was obtained by comparing an Inland test shot to my test shot at
the same location and calculating backwards. A sample of the CSTC was obtained for lab test of
maximum density per ASTM D-1557. All test locaiions but four of fifly-seven met or exceeded Spokaae
County staadards of 95% vompaction based on the approximate maximum density value obtained from
Inland's resuhs. Those four were 94% of maximum.

Curb reve,als appeared to be correct for 2" of class "B" asphalt concrete pavement per de#ail "B" on page 6
of plans. Contractor was adjusting manhole rim elevations at time of testing. STI will return to test
compaction of CSTC around manholes at a later date.

Carntractor was adjusting manhole nm elevahons and gmuting at time of arrival. CSTC had been
ba.ckElled around completod manhales but was aat going to conduct any compaction aciivities unti1 the
grout has set up. Two compleW manhales were tested by soil probe. Resutts showed that CSTG Uad
been p1aced araund the manhole wrth na water added. Contractor said that the manholes would, be ready

Arrived on site to observe laying of one hft o~f aspha.lt. Upvn am'ual, the tack caat had been applied to al,l
curbs and exssting vert.ical pavement. Existing asphalt had been freshly savcr cut. Paving crevv began
laying asphalt at south end of ~~elley Lake Lane. The asphait was latd according ta design specifications,
which called fnr a2 inch 1ift. Sarrtple af asplalt was cvllected on ShelXey Lake Lane at Stn, 19+I0. Field
tem,perature afapproxi'Ltnately 275° F met specificatians, along with a subgrade temperature of 69'.
Using a maximiun density of IS 1.4 lbfW, the asphalt density ranged from 88°/fl to 96°lo. Since not all the
asphalt met density specifications of at least 92°l~, crevvs reralled problem areas. Upon rera'lling, ruests of
asphalt met os exceeded minimum spocificatians.
