PROPERTY NAME ACTION TAKEN 11 Extinguishment by fire service personnel OFFICER IN CHARGE Battalion Chief Warren Kennett Valley 20 INCIDENT NARRATIVE E6, PL1, R1, M1 and V20 code response to the area of Jackson and Vista. Reported residential structure fire. En route dispatch advised that smoke and flame were showing from the rear of the structure. Subsequent to the update, V20 upgraded the incident to an 11W. The upgrade brought E5, PL7, AMR, V32 and V41. Upon arrival E6 found a working fire in a single story wood frame residence. E6 initiated command and directed PL1 to take the hydrant. A bystander advised E6 that someone may still have been inside the residence. Command directed R1 to conduct a primary search of the basement upon arrival. PL1-3 and E6-3 initiated and interior attack from side Bravo. PL1-1 and PL1-2 followed the attack team into the residence with a back-up line. M1 was directed to establish RIT. Upon arrival V20 assumed command and E6-1 was made Division A. Command directed E5 to supplement RIT. PL7 was directed to continue in and standby the CP. SCSO advised that they had established traffic control. SCSO also advised that an occupant of the residence was in need of medical attention for burns on his forearms. AMR was directed to initiate treatment and they were later assisted by M1 personnel. For further patient information see PCR and civilian casualty report. The fire was quickly knocked down by the initial attack personnel. Primary and secondary searches revealed no one was inside. A third and final search also revealed that no one was inside the residence. PL1 and E6 personnel were sent to rehab. E5 and PL7 personnel entered the structure to begin overhaul operations. R1 was directed to set up rehab. Overhaul operations were suspended to allow V41 to conduct a cause and origin investigation. V41 advised that he would be on scene for several hours and would need the assistance of one or two apparatus. Command directed Dispatch to respond E2 to the incident to and was assigned with E6 to assist V41. Avista responded and shut off power to the residence. PL1, PL7, E5 and M1 were returned to service. R1 was returned to service after they had completed filling SCBA cylinders. V20 returned to service and transferred command to E6. SVFD personnel were rehabed on scene per SOP. No fire fighter injuries were reported. Cause and origin investigation by V41. The cause of the fire is not known at this time. The structure and its contents appear to be a total loss. APPARATUS RESPONDED Engine 6 Responder Narrative Capaul, 211, Eng 6 supplemental report Eng 6 responded code from station 6 to a report of a structure fire at the corner of Jackson and Vista roads. Upon our arrival we found a heavily involved 1 story wood framed residence. Fire was blowing out the front and back windows. Eng 6'crew advanced an 1.75 line to the front door to begin transitional attack. PL 1 crew assisted with the attack and also provided manpower for water supply. Medic 1 was assigned RIT, R-1 was assigned S&R of the basement as we received a report of possible victims in the basement. Nothing was found. V-20 arrived on the scene and assumed Vista Command. After fire was knocked down we began overhaul. V-42 arrived on the scene. Overhaul was halted to allow for an investigation. Crews completed overhaul after V-42 completed his work. Eng 6 returned in service. Eng 2 was left at the scene for further overhaul. Ken Capaul, Capt. VE2 crew: Barstad, Cassano, Sanchez Steven D Barstad 180 APPARATUS RESPONDED Valley 43 Responder Narrative MehaffeyC, 239 narrative V43 responded to 2609 N. Vista from 1-90 and Geiger road, structure fire. On arrival, I reported to BC Kennett and then to V41, DFM Loftin. I assisted V41 in the fire origin and cause determination of the fire. Clifton Mehaffey Fire Inspector Spokane Valley Fire Department APPARATUS RESPONDED Valley 41 Responder Narrative LoftinN, 190 - Response Remark V41 was at Station 1 at the time of the fire dispatch and started that direction upon hearing the report of'flames out the back of the house'. While enroute to the scene V20 upgraded the call to a working fire and I received the page at Knox and Argonne. On my arrival at the scene I met with V20 and immediately requested additional investigative assistance from V43. See the Investigative Reports for additional details. **N.Loftin**190** APPARATUS RESPONDED Valley 42 Responder Narrative For Miller, K cancelled Cancelled, V41 responded and investigated the fire with V43. APPARATUS RESPONDED Medic 1 Responder Narrative smetzlerj VM1 responded emergent from station to the above address for a fully involved house fire. A column of dark colored smoke, in the area of the fire, could be see while en route northbound on Argonne. VM1 arrived to find smoke and flames coming from the A side of the building. VM1 was assigned RIT. AMR medic stated that they had evaluated the pt and were awaiting the pt's decision as to weather or not he would allow AMR to transport him to a hospital; the AMR medic stated that we were not needed so we returned to service. Upon arrival at stat, V20 requested that we return to the scene to reevaluate the pt and to evaluate the father of another occupant of the involved house. See PCRs for details. VM1 was cleared from the scene and returned to service. Gese Smetzler APPARATUS RESPONDED Pumper Ladder 1 Responder Narrative Olson, E, 187, PL-1 Report PL-1 responded code from station to N. 2609 Vista Road, structure fire. Enroute, V-20 upgraded to a 11W as smoke was reported visible by 911 callers. E-6 arrived on scene about 30 seconds prior to PL-1 and we were directed to split our crew with E-6 and provide attack and backup. The attack crew, made up of third men from E-6 and PL-1, went in through the open garage door into the house on the main floor and knocked down the fire in the living room and front bedroom. The backup crew, made up by the officer and FEO, went in behind the attack crew and protected their exit and began checking for extension. At this time, Command notified us that a person may still be in the residence so the attack team went to the basement for primary search and the backup team continued with primary search on the main floor. Nothing was found and both crews exited the building for Rehab and new bottle exchange. After Rehab, we were re-entered the basement to conduct secondary search and again, nothing was found and we exited the building. Command then directed PL-1 to vertical ventilate the building and we accomplished same by cutting a vent hole on side C near the roof peak to allow accumulated heat and smoke to be removed. By this time, an Investigator was on scene conducting an origin and cause investigation - see his report for C&O determination. We put our gear back on the PL and assisted other units in picking up hose before being released from the scene to return to service. E. For a reported house fire. Upon my arrival I positioned V32 Van to block Vista from Trent for traffic control. I reported to V20, And began my Scene Safety Survey. The power to the back of the home was still energized with most of the fire knocked down. There was no gas connection to the home. But there was a strong odor of gas on the back porch. All the main walls were sound as were the rafter system in the roof. As overhaul continued I placed more cones on Vista to the north for traffic control and provided water to rehab. Personnel were going through rehab on a regular bases. Also caution tape was placed completely around the structure. Power was secured be Avista. V32 returned to service. Written by Jeff Bordwell V32 APPARATUS RESPONDED Pumper Ladder 7 Responder Narrative Cruger T# 198 VPL-7 to 2609 Vista on a 11-W code from station. Upon arrival VPL-7 was directed by Vista Command to pack up and report to the command post. VPL-7 was directed to replace VE-5's crew and enter the building and commence overhaul. VPL-7 entered the building from the garage entrance and proceeded to a bedroom on the A-D corner where there was a small amount of fire on the floor. When we entered the bedroom we found flames floating on the water that was on the floor. Clothing that was in the water on the floor was collected by VPL-7-1 and taken to the C side of the building where an inspector was waiting. VPL-7 extinguished the fire in the A-D corner of the house by lifting the bed up so all sources of ignition could be extinguished so that the flammable liquid on the floor would not ignite. A closet door was opened in the bedroom on the A-D corner of this house and clothing on hangers ignited when it became exposed to oxygen. A piece of this clothing was taken to side C where it was collected by an inspector. VPL-7 was directed to stop overhaul in the house pending investigation. VPL-7 exited the building and rehabbed waiting for re assignment. VPL-7 was assigned to assist VE-6 with loading their hose with that complete VPL-7 was released by Vista Command. Upon arrival VR1 was directed to do a primary search of the basement using a TIC. Nothing was found so we did a primary search of the main floor as well. After completing the primary search we were assigned to extinguish a small basement fire. Division A then assigned us secondary search in the basement as there was a report of a possible victim downstairs. Nothing was found. We were then instructed by V20 to set up rehab after changing out our air bottles. For the remainder of the incedent, we filled bottles and ran rehab until it was time to load hose on Engine 6. En route Dispatch advised that SCSO was on scene and reporting visible fire from the rear of the structure. I directed dispatch to upgrade the incident to an 11W. Upon arrival found units on scene of a single story wood frame residence. A large volume of dark smoke and visible flame were emanating from the structure. SVFD personnel were had made entry through a door on the Bravo side. R1 personnel were entering the structure from the Bravo side to conduct a search of the basement. I contacted Vista Command/Capt. Capaul. He advised that bystanders were advising that someone may have still been inside the structure. I advised him that I would take command and that he would take Division A. Vista Rd. I responded from my home. Upon arrival I met with BC Kennett. He advised me that this was most likely a rekindle of the fire from the night before in which Norm Lofin had arrested an individual for setting the fire( report#04-427318). I consulted with Norm Loftin by phone and asked if he wanted to come to the scene or hold it for him. He said he did not need any more evidence other that photos. FIRE SCENE: The single story house with a basement had the roof burned away over north half of the living room and the bathroom and NE bedroom. One of the rafters over the bath tub was completely consumed. This was most likely the area the fire continued. According to fire crews the original fire had damaged mostly the living room, NW bedroom and the attic. The fire continued in the attic area above the bathroom. All the ceiling area was removed prior to our departure. FIRE CAUSE: This fire was a continuation of the arson fire from the previous day. Cellulose insulation in the probably trapped heat from the previous fire. EVIDENCE; I took photographs of the fire damage with my 35 MM camera and also had them saved to a computer disk.
