General Overview - The owner Valley General Hospital is proposing the construction of a 1,818 square foot addition for medical records. Also proposed is the interior remodel of the radiology department. SUMMARY OF DEPARTMENTAL REQUIREMENTS: I. Planning Department (456-2205) A. Discussion/Requirements 1. Comments were submitted to the applicant and the Division of Buildings by Lorne Lakshas. _ .1 _ i 2. The parcel number submitted is incorrect. The correct parcel number is 15542-0605. 3. The proposed is located in an UR-22 Zone. 4. The parcel of land illustrated on the site plan does not reflect the correct boundaries of the parcel. Please include a logo on the revised site plan illustrating the correct boundaries of the parcel. 5. A certificate of exemption to clarify the boundaries of the parcels may need to be obtained for the subject property. 6. The required number of parking stalls for the proposed has not been determined. Lorne Lakshas will evaluate the project and get back to the applicant with the number of required parking stalls. 7. Please submit a revised site plan indicating the square footage and usages in the proposed and existing structures, a logo illustrating the correct boundaries of the parcel and the number of beds located in the hospital. B. Permit Release Requirements 1. Please submit a revised site plan as described above for review and approval. II. Division of Engineering & Roads (456-3600) A. Discussion/Requirement 1. Comments were given to the applicant and the Division of Buildings by Bob Turner. 2. The roof will be a non-asphaltic material therefore, all roof drainage may go directly into a drywell. B. Permit Release Requirements 1. The Division of Engineering & Roads released their portion of the permit with the condition that the roof be of non-asphaltic material and that there be no new hard surfaced areas. - 2 - S i 01/ /PI III. Health District (456-6040) A. Discussion/Requirements 1. The proposed will be connected to the public sewer system. 2. All construction debris is to be taken to a licensed landfill. 3. The water purveyor for the site is Modern Electric Water Company (456-8000) . B. Permit Release Requirements 1. The Health District has released their portion of the building permit. IV. Fire District No. 1 (928-2462) A. Discussion/Requirements 1. The proposed above ground storage tanks need to be protected from the public by a fence. Also, a sign needs to be placed on the fence indicating what is in the tanks. Please illustrate fence and sign on the site plan. V. Division of Buildings (456-3675) A. Discussion/Requirements 1. Plan review has been completed on preliminary plans only by Ken Jeffrey. 2. Separate permits are required for the two above ground tanks. Please indicate on the plans that the tanks -are above ground tanks. 3. All welding shall be done by an approved fabricator or special inspections will be required on all steel. Special inspections forms were given to the applicant during the meeting. A designated place shall be provided on the .site where all the special inspection records can be kept. 4. Please submit shop drawings for the sprinkler system that is to be extended into the addition for review. - 3 - 010 5. The plumbing contractor shall contact the Division of Buildings prior to any construction. 6. The mechanical equipment curb shall be of non-combustible material. 7. Please submit a critical materials list to the Division of Buildings for review. B. Permit Release Requirements 1. Two complete revised sets of plans addressing the items discussed during and after the meeting needs to be submitted for review and approval. APPLICANT'S COMMENTS/ACKNOWLEDGEMENT: The applicant acknowledged that all comments/requirements were understood and had no further questions at this time regarding permit requirements. CORRECTIONS: To minutes or changes from what appears in the minutes, we should be notified within one day of your receipt of these minutes. To site plans as required by any or all of the above-listed departments shall be submitted to the Division of Buildings who will then distribute accordingly to those departments. ADJOURNMENT: There being no further business, Tom Davis thanked the applicant and the departments for their participation and reminded all that he should be contacted if there are any unforeseen problems which may arise. Comments so far would indicate these conferences are successful and do enhance the permit system.
