SCOPE,/PROJgCT DESCRIPTION:
I. General Overview - Proposed is the construction of a single
dentist building. With the basement being unfinished, the entire
operation will be located on the first floor. Total staff for the
operation will be approximately 7 to 8 people. In the future, the
basement may be leased to a separate party.
SIIMMARY OF D$PARTMENTAL REQIIIREM$NTB:
I. Planning Department (456-2205)
A. Discussion/Requirements
1. The site is zoned Residential Office (RO) and is
subject to zone change, ZE-5-79.
2. Zone change, ZE-5-79, does impose certain conditions,
such as landscaping, location of parking, signage, and
setbacks.
3. The sign depicted on the site plan needs to be more
detailed; i.e., square footage, height.

All signage shall conform to the standards of the RO zone
and a separate permit will be required.
4. The site plan needs to illustrate the center line of
the roadways.
5. There shall be no parking stall located more than 60
feet from the landscape feature area which shall be not
less than 32 square feet in area.
6. Fencing is required for the proposal.
7. The basement is not to be occupied. If in the future
occupancy is intended, the available parking at that time
will have to be considered. (Parking for the dental
building is based on 1 parkinq space per every 170 square
feet of floor area).
8. The applicant indicated that that adjacent property
may be acquired and used for off-street parking. The zone
of the adjacent site needs to be verified to see if
off-street parking is allowed.
• B. Permit Release Requirements
1. A revised site plan illustrating sign details, center
line of roadways, and required landscaping/parking. Also,
a landscape plan along with a promissory letter from the
owner stating-the landscaping will be installed shall be
submitted for review and approval.
II. Engineering Department (456-3600)
A. Discussion/Requirements
1. Comments were given to the applicant and Building and
Safety by Bob McCann.
2. An approach permit for Pines Road does not need to be
obtained from the Washington State Department of
Transportation since access is not going to be utilized.
3. It was stated that a bond may be filed in lieu of
completing the required road improvements prior to the
release of the building permit. There are three (3) types
of bonds, one being a"Performance Bond," the second being
an "Assignment Bond," and the third being an "Irrevocable
Letter of Credit."
4. Your contact person for the Engineer's Department is
Jerry Sims.

r + B. Permit Release Requirements
1. A drainage plan shall be submitted for review and
approval.
2. Right-of-Way dedication, as per the zone change,
ZE-5-79, shall be completed.
3. Road and drainage plans for road improvements on 8th
Avenue must be submitted for review and approval.
4. Construction of required road improvements or filing
of a bond shall be completed prior to the release of the
building permit.
5. An approach permit for 8th Avenue shall be obtained
prior to the release of the building permit.
III. Health District (456-6040)
A. Discussion/Requirements
1. A new on-site sewage system has been applied for and
• the test holes have been excavated and approved.
2. The residence has been demolished and removed. The
septic tank has been pumped and removed.
3. A ten (10) foot separation shall be maintained between the sewage disposal system and the 11208" areas.
4. The sewage system permit will be ready to issue after
verification from the Utilities Department that double
plumbing has been installed and .from the Engineer's
Department that the 11208" areas are not in conflict with
the sewage disposal system are received.
B. Permit Release Requirements
1. Verification from the Utilities Department regarding
the double pluntbing and the Engineer's Department
regarding any 11208" conflict shall be received prior to
the release of the building permit.
IV. Building and Safety (456-3675)
A. Discussion/Requirements
l. Revised plans were submitted prior to the meeting and
Ken Jeffrey did not complete the plan review but stated
that they appeared to be okay.

1. There is some concern with the fill that was placed
where the existinq house was. The applicant indicated
that the new building footings are to be deeper than the
existing house basement footings. Verification of fill
shall be made prior to the pouring of the footings.
2. The amount of fire hydrants for the site are
adequate.
3. Relocation of the medical gas storage area is
preferred by 8uilding and Safety, because if the basement
is ever occupied, the medical gas storage area is located
near a potential primary entrance. The Uniform Fire Code
requires that no medical gas tanks be located near an exit
area. Other locations or options should be considered.
B. Permit Release Requirements
1. Final review of the revised site plans shall be
completed.
APPLICANTIB COMMBNTS/ACKNQWLEDGEMENT:
The# applicant acknowledged that all comments/requirements were
understood and had no further questions at this time regarding
permit requirements.
CORRECTIONB:
To minutes or changes from what appears in the minutes, we should be
notified within one day of your receipt of these minutes.
To site plaas as required by any or all of the above-listed
departments shall be submitted to Building and Safety who will then
distribute accordingly to those departments.
ADJOIIRNMENT: There being no further business, Jeff Forry thanked
the applicant and the departments for their participation and
reminded all that he should be contacted if there are any unforeseen
problems which may arise.

A. INTRODUCTION
This matter having come before the Examiner Committee on April 20, 1979, and
the members of the Committee present being Ken Kennedy, Abby Byrne, and
John Culler and there being the following person present who wishes to request
approval of a Zone Change; Mrs. Jeckle for a Zone Change, ZE-5-79,
Agricultural Suburban to Residential Office for the purpose of an office building.
B. FINDINGS OF FACT
1. That the existing land use in the area is residential and multi-family.
2. That the Comprehensive Plan designates this area as residential.
3. That the existing zoning of the property described in the application is
Agricultural Suburban.
4. That the provisions of RCW 43.21C (The State Environmental Policy Act)
have been complied with.
5. That the proper legal requirements for advertisement of the Agenda Item
