We have found the plans to be complete and are issuing our approval with the following conditions: • Based on your menu and food prep steeps a commissary will be necessary.An application for the commissary is included with this letter. • The source and name of the potable water supply was not identified on the plans. Please provide the information for your source of water. Fresh water hoses are required to be food grade quality. • The location and address of the wastewater disposal site was not identified. Please provide the information for wastewater disposal. • The proposed site location was not noted. Please provide the information for your actual site location. • Restroom facilities must be available within 200 feet of your site location. Please provide restroom facility information for Food workers. We will be notifying the appropriate building department of our approval in order for you to obtain your building permits and commence construction. Before proceeding, please contact Washington State Department of Labor to determine what permits may be required for construction, plumbing, mechanical or fire suppression work. A fee of$110.00 per hour is charged for the plan review and pre-occupancy inspection. An invoice will be given to the person in charge at the time of the pre-occupancy inspection. The fee is to be paid within 30 days of receipt of invoice.A$100.00 late fee will be applicable if not paid within the 30 days. The current fee for a Mobile Food Unit is $550.00; it may be adjusted at the time of opening. Applications(s) are to be submitted at least fourteen (14) days prior to the projected date of opening to avoid paying a double permit fee. No applications(s)for permit(s)shall be accepted less than three (3)days prior to the date of opening due to the time needed to review the application. r After you have completed construction,the Health District will conduct a pre-opening inspection. At the time of this inspection it will be verified that the construction is in compliance with the approved plans. (Any changes in menu or equipment shall be pre-approved by the Health District prior to construction.) Any equipment installed in the establishment is to be NSF approved or equivalent. Any home-style equipment found on the premises will have to be removed. The permit to operate will be given to you at the end of the pre-opening inspection if the construction is found acceptable and all the other appropriate agencies have given their approval.The issued permit is required to be posted on your mobile unit. To insure that you obtain your pre-opening inspection in a timely manner we request that you contact us at least three(3)days before you want an inspection to arrange it in our schedule. All employees handling unwrapped food are to obtain a food worker card within 14 days of employment. Food and Beverage training is now available at www.srhd.org/safetv/food.
