Introduction: The State Environmental Poltcy Act of 1971, Chapter 43.21C. RCWt requlres all state and local
governmentat agencies to consider environmental values both far thetr own actions and when licensing private
proposals. The Act also requires that an Environmental Impact Statement be prepared for all major actions
significantly (and "adversely", as per WAC 197•10) affecLing the quallty of the physical environment. The
purpose of this checklist 1s to help the agencies involved determine whether or not a pcoposal is such a
major action.
Please answer the following questions as completely as you can with the informatlon presently available to
you. Please answer questfons as "yes" or "maybe" if, in your opinion, even only slight impacts will result.
The reviewers of the checklist will be aware of and concern themselves with the de ree of irtpact, asking you
fmr more information, if necessary. Where explanations af your answers are requ re , or where you believe
an explanatian would be helpful to government decision-makers, include your exptanation in the space provided,
or use additlonal a es, if necessary. You should include references to any reports or studies of which you
are aware and wh ch sre relevant to the answers you provide. Complete answers to these questions now wi11
help all agencies involved rrith your proposal to undertake the requlred environmental review without unnec-
essary delay.
The following questions apply Lo your total praposal, not just to the license for whlch you are currently
applying or Lhe proposal for whlch approval is sought. Your answers should lnclude the impacts which will be
caused by your propasal when it is completed, even though campletion may not occur until samettme in the
future. This wi11 ailcw a11 of the agencies whtch will be involved to complete their environmental review
now, without duplicating paperwork 1n the future.
No application sha11 be processed untii the checklist has been completed and returned Lo the appropriate
County department. State law requires ex lanations for ever es" and "ma~Ybep aaswer on the checklist.
The person completing the torm rtay be reqred to provQe expTartation for rto answers, and"ln same cases,
more detatled informati.on to aid in a threshold determination.
NOTE: This is a stanaard form being used by all state and local agencies in the State of Washinqton for
various types of proposals. Many of the questions may not apply to your proposal. If a question
does not apply. just ansrrer it Ono" and continue on to the next question.
