KNOWI ALL MEN BY THESE PRESENTS, That park Road Church of God, an unincorporated
body with Headquarters in Cleveland, Tennessee,
being the owners of the following described lande in consideration of Mutual
senefita to be hereafter derived do for themselves, their heira, grantees, assigna
and successor(s) in intereet hereby agree to authorize Spokane County to place their
name(e) on a petition for the formation of a Road•Improvement Diatrict (RID) by the
petition method pursuant to Chapter 36.88 RCW, which petition includea the owner(e)
property, and further not to objeot, by the signing of a ballot, or not to protest
to the formation of a RID by the resolution method pursuant to Chapter 36.88 RCw,
which reaolution includea the owner(e) property. If a RZD ie formed by either the
petition or resolution method,.~.as'provided for in Chapter 36.88 RCW, the owner(s) or
successor(s) further agree: (1) that the improvementg or conatruction contemplated
within the proposed RID are feasible, (2) that the benefits to be derived from the
formation of the RID by the property included therein, together with the amount of
any County participation, exceeds the cost and expense of formation of the RID, and
(3) that the property within the progosad RID is sufficiently developed. Provided,
further, the owner(e) or successor(s) shall retain the right, as.. suthorized under
RCw 36.88.0901_--to object to any assesement(e) on the property ae a result of the
improvemente called for in`conjunetion with the formation of a RIO by either the
petition or resolution method under Chapter 36.88 RCW and to appeal to the Superior

successor(s) agree to conatruct the required improvemente 8t their own expensej pay

Department of Buildings Code Compliance Coordinator, Tom Davis, with the following in

SCOPEIPROJF,CT DESCRIPZTON:
I. General Overview - The owner, Associated Industries of the Inland Northwest, is
proposing to remodel a 8,100 square foot structure. The proposed will be a change in use to
office space from the previous use of the existing building.
S Y OF DEPARTACENTAL REQ
I. Planning Department (456-2205)
• . A. Discussion/Requirements •
1. The proposed is located in a B-1 zone. The proposed use is allowed in the
zone.
2. The proposed is located on parcel number 12531-6501 and 6502. The
involved parcels will need to be aggregated into one parcel through a Certificate
of Exemption. Forms are available from the Planning Department for complet-
ing this.
3. The west property line on the submitted site plan is incorrect. A revised site
plan needs to be submitted with the conect west property line.

4. The parking as illustrated on the site plan is adequate. 23 spaces are required
and 40 spaces are shown.
5. If the valuation of the remodeling is greater than 50 % of the valuation of the
existing building, additional landscaping will be required. The architect indicat-
ed that the cost of remodeling would amount to approximately 1/3 of the valua-
tion of the existing building. Although,this would mean that Planning could not
require additional landscaping, they would like to see landscaping added to the
proposed.
B. Permit Release Requirements
1. Please submit a revised site plan including a logo illustrating the conect
boundaries of the parcel for review and approval.
2. Please submit a Certificate of Exemption aggregating the two parcels into one
parcel.
II. Division of Engineering & Roads (456-3600)
A. Discussion/Requirements
1. Comments were given to the applicant and the Department of Buildings by
Bob Turner. 2. Please submit a drainage plan for the site that is in compliance with
Spokane County Guidelines for Stormwater Management if any new asphalt is
added. It was noted that resurfacing does not constitute new asphalt.
3. The proposed shall comply with conditions of the Zone Change ZE-15-65.
4. A RID/CRP Agreement, Notice to the Public No. 6, needs to be recorded for
Park and Utah. Please submit a copy of the recorded property deed to the
Division of Engineering & Roads so that the agreement can be prepared.
5. An additional 10 feet of right of way along Park needs to be set aside for
future road expansion.
6. An Agreement to Pay Fees was given to the applicant during the meeting.
The agreement is to be signed by the owner and returned to the Division of
Engineering & Roads. Plan review will not be completed until this document is

B. Permit Release Requirements
1. Please submit a a recorded property deed, so that a RID/CRP Agreement,
Notice to the Public No. 6, can be prepared.
2. Please submit a copy of the Agreement to Pay Fees signed by the owner.

III. Health District (456-6040)
A. Discussion/Requirements
1. The water purveyor for the proposed is Orchard Avenue Irrigation District
No. 6 (926-4563).
2. Please submit a critical materials list to the Department of Buildings for
review.
3. All construction debris shall be taken to a licensed landfill.
4. Proposed must connect to the available sewer. Please locate the septic tank
located on the property and properly abandon it.
5. Applicant was advised that the Mica Landfill is the only local landfill accept-
ing asbestos, and they will be closing December 31, 1991. If any asbestos is
going to be removed from the proposed, it would be advisable to do it prior to
the end of the year.
B. Permit Release Requirements
1. Since the proposed will be connecting to public sewer, the Health District is
ready to release their portion of the building permit.
VI. Division of Utilities (456-3600)
A. Discussion/Requirements
1. Proposed must connect to the existing sewer by direct access. Applicant was
given a profile map of the sewer line. A sewer permit will be required.
2. If any work is done in the county right of way, a permit will be required.
3. Please contact Bill Johns or Jim Leget for further information regarding connection requirements.
B. -Permit Release Requirements "
1. Please submit a sewer plan, a sewer connection agreement, and a sewer
connection permit. By signing the sewer agreement, the applicant agrees to: 1)
. pay a monthly. charge, 2) pay General Facility charge, not: object to any futuFe ULID.
V. Fire District No. 1(928-2462)
A. Discussion/Requirements l. The existing fire hydrant on the southeast corner of Park & Utah will need to
be changed from a two port to a three port hydrant. Please contact Mike Cline
with the water purveyor (Orchard Avenue Irrigation District No. 6 at 926-4563)
for installation requirements. The estimated cost is approximately $3,000.

2. A lock (Knox) box entry system for fire department personnel access is
required to be installed. Forms for ordering was given to the applicant. The
completed forms are to be returned to the fire district. The box shall be installed
prior to occupancy.
VI. Department of Buildings (456-3675)
A. Discussion/Requirements
1. Aggregation of the parcels into one lot is required.
2. Please submit a critical materials list for review.
3. If construction specifications require concrete with a design strength over
2,500 p.s.i., high strength bolting, or structural welding special inspections will
. be required on the proposed. Please indicate the name of the testing laboratory
and submit copies of the inspection reports to the Division of Buildings for
review.
4. The proposed shall meet the minimum requirement of the Regulations for
Barrier-Free Facilities. Specifically, a ramp from the parking lot to a primary
entrance of the building shall be not steeger than one vertical to 12 horizontal in
the direction of travel, nor one vertical to 48 horizontal transverse to the direc-
tion of travel.
5. All welding shall be done by an approved fabricator or special inspections
will be required on all steel. Special inspection forms are available from -the
Department of Buildings.
6. All floor drains and sinks must be blocked off.
7. Please submit a detailed floor plan of the proposed remodeling for review.
8. Drawings shall be directly stamped and signed by the architect. (Pen and ink ,
- not copy)
B. Permit Release Requirements
1. Please submit verification of the aggregation.
2. Please submit a critical materials list.
3. Please submit a detailed floor plan of the proposed remodeling.
4. Two complete revised sets of plans addressing the items discussed during the
meeting need to be submitted for review and approval.

The applicant acknowledged that all comments/requirements were understood and had no
further questions at this time regarding permit requirements.
