UTILITY SERVICE AGREEMENT
I/We, the undersigned real property owners, request and authorize Vera Water and
Power ("Vera") to perform work generally described as: Installation of a 2"Water Service at
Blake on the North Side of the RRROW. The City of Spokane Valley will mark the location for the meter Vera
Water&Power will install the Service and all associated appurtenances.
at this address: Approximately 13506 E. 2nd Avenue ("real property")
I/We understand that Vera will charge payment for this work at the real property based
upon actual time required and materials used, even if above or below a rough estimate
given.
I/We agree to pay Vera the total of charges for this work within thirty(30)days of billing.
I/We understand Vera will bill these charges separately from, and in addition to, the
regular utility accountforthe real property. If these separate charges are not paid when
due, Vera will treat the entire unpaid amount as a regular utility charge, subject to
regular Vera credit terms and remedies for nonpayment of utilities services, including but
notlimitedto DISCONNECTION OF UTILITYSERVICE,and FORECLOSURE OF LIEN
against the real property.
I/We agree and understand that the charges hereunder shall be considered as charges
under RCW 87.03.445.
I/We are authorized owner(s)of the real property or authorized agent(s)certified by
signature below.
I/we agree to the terms of this Utility Service Agreement set forth above and to rules and
regulations applicable to all Vera utility service customers.
