There was no base map survey of the project roadway surface. The project plans did not show 6 existing
manholes and 4 water valves that will need to be lowered prior to grinding and raised prior to paving. This
change order adds the following items to the Contract:

The required adjustment of the manholes is a result of several factors. First, the manholes are located
well within the existing 2 way center turn lane, so initially it appeared no adjustment would be required.
Second, the new 11.5 foot lane widths reduced the width of the center turn lane. Third, the grind limits on
each side of the center turn lane were adjusted further towards center so that C-1 inlay tape could be
installed away from a pavement edge. The manhole locations were not field verified against the grind
dimensions in the final design.
The total estimated cost for this Change Order work is $6,000.00 paid for by Fund 311. There are
sufficient funds in the project budget to cover these costs.
