That the individuals and parties named herein as having an
interest in the above described property or project agree to the
following:
1. Reimburse Spokane County for project review and inspection
• £ees as specified in Board of County Commissioner's
Resolution 80-1592 attachment C. as amended. The fees will
be based on actual salary costs incurred by Spokane County
for project reviews and / or inspections plus a ten percent
administrative charge, and will be billed monthly.as •
accrued.
2. The undersigned agrees that these fees aZe due and payable
upon receipt ot the billing as specified above. The
monthly billing should be sent to the attention of:

CALL TO ORDER: The meeting was called to order at approximately
10:00 a.m. by the Division of Buildings Code Compliance Coordinator,
Tom Davis, with the following in attendance.

SCOPE/PROJECT DESCRIPTION: I. General flverview - The owner Valley Young Peoples Clinic, is
proposing a 24 x 40 addition to the existing build.ing. The addition.
will consist of physicians offices, and a conference room. The.
existing building will be remodeled and will contain 10 pediatric
exam rooms. .
SOMMARY OF DEPARTMENTAL REOIIIREMENTB: I. Planning Department (456-2205) A. Discussion/Requirements 1. The proposed is located in a B-3 zone.
2. A copy of the Findings and Order for zone change
ZE-48-88 was given by Lorne Lakshas to the applicants.
3. All traveled surfaces are required to be paved and be :
within 60 feet of a landscape feature consisting of a

minimum of 32 square feet in area.
4. If there are any signs proposed, the height, width,
and size is to be shown on the site plan.
5. A 5 foot strip of Type III landscaping is required
along the road frontage. A copy of landscaping
requirements were given to the applicant during the
meeting.
6. A Certificate of Exemption shall be obtained from the
Planning Department to aggregate the two involved parcels.
B. Permit Release Requirements
1. Please submit for review and approval a revised site
plan illustrating detailed landscaping.
2. An estimate from a nursery including size, type and
quantity along with a promissory letter from the owner
stating the landscaping will be installed shall be
submitted.
3. A Certificate of Exemption shall be obtained from the
Planning Department to aggregate the two parcels.
II. Division of Engineering & Roads (456-3600)
A. Discussion/Requirement
1. Comments were given to the applicant and the Division
of Buildings by Bob Turner.
2. Please submit a drainage plan for all new hard
surfaced areas that is in compliance with Spokane County
guidelines for stormwater management.
3. Please submit a traffic and circulation plan.
4. The applicant shall construct a six foot wide
portland cement concrete sidewalk along the property
frontage on Mayhew road. Please submit plans for the
sidewalks prepared by a licensed engineer for review and
approval. Al1 required construction shall be completed
prior to permit issuance or a bond in an amount estimated
by the County Engineer to cover the cost of construction
shall be submitted. The applicant was advised to submit
the sidewalk plans as soon as possible so the issuance of
the building permit would not be delayed.

5. Approach permits shall be obtained for new driveway
approach along the county right of way.
6. An Agreement to Pay Fees was given to the applicant
during the meeting. The agreement is to be signed by the
owner and returned to the Division of Engineering & Roads.
Plan review will not be completed until this document is
executed.
B. Permit Release Requirements
1. Please submit a drainage plan for the site as
described above for review and approval.
2. Please submit a parking and traffic circulation plan
for Division of Engineering & Roads records.
3. Please submit sidewalk plans prepared by an engineer
for review and approval.
4. Obtain approach permits for new driveway approaches
along county right of ways.
5. Submit a copy of the Agreement to pay fees signed by
the owner.
III. Health District (456-6040)
A. Discussion/Requirements
1. The existing building is connected to an on site
sewage disposal system. The Health District has on record
a letter from the previous owner stating that the building
will be connected to the public sewer system prior to a
new occupancy. The applicant stated that they are
intending to connect the proposed to the public sewer
system. Please contact the County Division of Utilities
(456-3604) for information regarding sewer connection and
fees.
2. Daryl Way recommended that the building be tested for
radon gas prior to construction.
3. The applicant was informed that new regulations for
disposing of infectious waste are in the process of being
drafted and adopted.

4. All demolition and construction debris is to be taken
to a licensed landfill.
5. The water purveyor for the proposed is Vera Water &
Power District (924-3800).
B. Permit Release Requirements
1. The County Health District has released their portion
of the building permit.
IV. Fire District No. 1(928-2462)
A. Discussion/Requirements
1. Fire f low for the proposed is 2,000 g.p.m.
2. One f ire hydrant is required to be installed. The
f ire hydrant shall be installed as per an approved water
plan. The water plan shall contain the location of the
hydrant, hydrant assembly detail, and plumbing detail.
The water plan shall be submitted to the fire district and
water purveyor for approval prior to installation. There
are two adequate locations to install the f ire hydrant, it
will be up to the applicant to decide which location to
install the hydrant.
3. A lock (Knox) box entry system for f ire department
personnel access is required to be installed.
V. Division of Buildings (456-3675)
A. Discussion/Requirements
1. Plan review has been completed on the site plan only
and a letter was sent to the applicant by Tom Postlewait.
2. Please submit a letter to the Division of Buildings
stating that no surgery is conducted in the existing
building and the proposed addition.
3. The two involved parcels need to be aggregated into
one parcel to eliminate the property line which passes
through the proposed addition. .
4. The proposed shall meet the minimum requirements of
the Regulations for Barrier Free Facilities.
5. Please submit a critical materials list to the

Division of Buildings for review.
6. Separate restroom facilities shall be provided for
each sex when there are four or more employees and both
sexes are employed.
7. Cross connection devices are to be installed on
equipment which will cause pollution of the domestic water
supply.
8. Special inspections are required on concrete with a
design strength of over 2,500 p.s.i. Please indicate the
name of the testing laboratory prior to the release of the
permit. Also, submit copies of the testing reports to the
Division of Buildings for review.
9. Drawings shall be directly stamped and signed by the
architect.
B. Permit Release Requirements
1. Two complete revised sets of plans addressing the
items discussed during and after the meeting and other
items from the plan review letter needs to be submitted
for review and approval.
APPLICANT'8 COMMENTS/ACRNOWLEDGEMENT:
The applicant acknowledged that all comments/requirements were
understood and had no further questions at this time regarding
permit requirements.
CORRECTION3:
To minutes or changes from what appears in the minutes, we should be
notif ied within one day of your receipt of these minutes.
To site plans as required by any or all of the above-listed
departments shall be submitted to the Division of Buildings who will
then distribute accordingly to those departments.
ADJOURNMENT: There being no further business, Tom Davis thanked the

all that he should be contacted if there are any unforeseen problems
which may arise.
