To Whom It May Concern:
Per your telephone conversation with our office on January 11, 1995 your request for an
extension of your permit number 94004071 for the above -referenced property is approved
through July 11, 1995. If inspections to verify progress have not been requested within the
above time frame, a new permit must be filed, and appropriate fees paid.

Our records show that the above permit for work located at 13727 E EVERETT AVE,
SPOKANE WA has had no called inspection or other reported activity over the last 150 days.
Permits become void if the work authorized is suspended or abandoned for a period of 180 days.
In addition, the Uniform Codes require that there be a final inspection and approval on all
permitted work prior to occupancy or use. If the work authorized has not been suspended
and/or abandoned, please telephone the Division of Buildings at (509) 456-3675 before January
18, 1995 to schedule an inspection -- our inspection request line is available 24 -hours a day.
Failure to do so may result in payment of additional fees to reinstate the permit.
A recheck of our files will be conducted on the date indicated above. If our office is not
contacted prior to this time and we are unable to verify the status of the work, your permit will
be considered void. Should the continuation or completion of work be verified then further
compliance action may be necessary.
