Statement of Financial Responsibility
As part of Spokane County Library District's community-based mobile services, you have
requested a card for your facility and/or activities coordinator.
No overdue charges will be assessed for late materials checked out on this card, but we do
request that items are returned by the due date.
Checkouts to your facility card are suspended if the balance owing on the account exceeds
$25.00 due to lost or damaged material.
To begin or continue borrowing privileges, we require the signature of the individual in your
facility that is authorized to issue payment for lost or damaged materials.
Materials are checked out to the facility, not individual staff members. The card is for the facility
and its residents to use. Please let us know if you have a new Activity Director and remember
to account for the items on the card prior to staff departure.

Spokane Valley Parks and Rec is scheduled for library deposits 3:00 PM on Mondays each week.
Occurrences that fall on holidays will be rescheduled in advance. If there is a conflict with the scheduled
dates or you need to cancel a visit for any reason, please call the Mobile Services Department at (509)893-
8240 or email mobileservices@scld.org.
We agree to the following package of services from July 11, 2022, through August 22, 2022:
• A drop off collection of materials for your clients to use throughout the month.
To make each monthly deposit successful for your clients, please:
• Have a facility contact person available during the visit in case there are questions.
• Have space available for us to put the monthly drop materials.
• Have items for return ready for pick up.

• We have Kendall May listed as the contact for your facility. If another employee assumes this duty
