This Contract is revised as follows:
Description of Work
Change Order No. 3
DRYWELL CATCH BASIN MODIFICATION —This change includes removal and waste
of 35 cubic yards of material and replacement of 35 cubic yards of drywell rock around
the existing drywell and new catch basin. A concrete cap was also placed above both
structures.
CATCH BASIN UTILITY CONFLICT - During the placement of Structure #61 a gas
main was in conflict with the installation of the cone on the new catch basin type 1. A
steel plate was placed on the catch basin barrel and backfilled. The structure above the
steel plate was then excavated and the steel plate was removed.
SEWER REPAIR (SIDE SEWER SERVICE) — Sewer services to businesses were
repaired due to excavation of new catch basins at (3) three locations. This work
included removal of contaminated soil, emergency repair and lost production.
EXISTING DRYWELL REPLACE CONE — (2) Two Existing drywell cones at structure
#'s 7 and 11 were found to be damaged. This work included the replacement of the
drywell cone, concrete cap and adjustment rings.
REMOVE EXISTING SIGNAL BASES —Old signal bases that remained at the Gillis
intersection were removed so ADA ramp improvements could be made at the four
corners of the Gillis intersection
Materials
The Materials shall meet the requirements of the 2008 Standard Specifications.
Construction Requirements
Work shall meet the requirements of the 2008 Standard Specifications and the special
provisions. The installation shall include all labor, materials, tools and equipment to
complete the work.
