Agreement Date Date Cog -14 CHANGE ORDER Page 2 of 3 Contract No: 09 -031 Change Order No. 6 This Contract is revised as follows: Description of Work During asphalt grinding operations on the Sprague Avenue Project areas of inadequate asphalt thickness were encountered. The project specified that 2.5 inches of asphalt be removed with a grinder and replaced with 2.5" of Hot Mix Asphalt. Existing areas along Sprague Avenue had only 2 to 2.5 inches of existing asphalt. Grinding operations in these areas completely removed all existing asphalt. Existing material in these areas will need to be removed and reconstructed with crushed surfacing and or asphalt. Two (2) Working Days will be added to the contract. 1) PAVEMENT REPLACEMENT CSTC /3.5" HMA - Pavement replacement in the outside eastbound lane between Bowdish Road and Pines Road will be replaced with crushed surfacing and 3.5 inches of Asphalt. Existing material will be removed and hauled off. 2) PAVEMENT REPLACEMENT 2.5" HMA - Pavement patching in the center turn lane and other areas along Sprague Avenue between University Road and Bowdish Road that were discovered during grinding operations will be paved with 2.5 inches of asphalt. Existing material will be removed and hauled off. 3) PAVEMENT REPLACEMENT MISC AREAS - Pavement patching in miscellaneous areas along Sprague Avenue between Pines Road and Evergreen Road that were discovered during grinding operations will be paved with 2.0 inches of asphalt. Existing material will be removed and hauled off. Materials The Materials shall meet the requirements of the 2008 Standard Specifications. Construction Requirements Work shall meet the requirements of the 2008 Standard Specifications and the special provisions.
