THE CONTRACT IS MODIFIED AS FOLLOWS: Background: Issue#1 The City determined that Hot Mix Asphalt(HMA)repairs were necessary since there was less existing asphalt within the roadway than anticipated.After the 2-inch planing on the south side of Mission Avenue according to plan,underlying base material was encountered.The repair included removing the substandard areas 6"below final grade, scarifying and recompacting the existing base and patching back with 4 inches of HMA prior to top lift. Issue#2 The City determined after potholing for utilities that existing utilities conflicted with the plan location of the new storm pipe at CB 3 at Sta. 5+55.80 to DW 2 at Sta.5+43.38. See Field Work Directive(FWD)#1. The storm was redesigned to use ductile iron and go shallower with the inverts to allow for the storm sewer to go over the top of the existing utilities and maintain the design intent. The abutting driveway approach next to CB 3 was determined to be badly cracked and deteriorated beyond repair so the City decided to remove and replace the driveway to provide a consistent neat edge for the new roadway paving and to accommodate the necessary drainage. Issue#3 There was a low spot in the existing curb at Sta.4+08.47 Rt creating ponding and deteriorating the existing pavement at that location. Per FWD#4,the City requested additional stormwater improvements to place a drainage structure near this low spot to collect the roadway runoff. In order to install the additional stormwater work, a concrete driveway had to be replaced. Description of Change: Issue#1 This change order compensates the Contractor for repairing the existing roadway where the existing HMA was not as thick as expected with underlying base material encountered and paving the section back 4-inch depth of HMA. Issue#2 This change order compensates the Contractor changes in material and the associated labor required to install the ductile iron versus the PVC pipe and the additional CDF backfill. This also compensates the contractor for additional quantity of removal and replacement of concrete driveway approach. Issue#3 This change order compensates the Contractor for the additional stormwater improvements associated with FWD #4. This also compensates the contractor for additional quantity of removal and replacement of concrete driveway approach. Specification: Requirement of Section 2-02, 5-04, & 8-06 of the Standard Specifications shall apply.
