THIS AMENDMENT No. 3 is made to the Agreement for Professional Services dated
Octobet20, 2009 by and between the City of Spokane Valley, a code City of the State
of Washington, hereinafter "City" and CLC Associates, Inc., hereinafter "Consultant'
and jointly referred to as "parties"
Pursuant to Section 1.0 Modifications of the Agreement for Professional
Services, the parties hereby agree to amend the Agreement as set forth in the
attached July 28" COSV request for additional Legal Descriptions, Exhibits and
Associated Work. These services will be tracked and billed on time and
materials basis, and are subject to all of the conditions set forth in the original
Agreement. The City agrees to pay Consultant up to $29,947.00 for the
additional services provided for in this Amendment to the Agreement.
Exhibits to this Amendment are as follows:
1. Professional Services Agreement dated October 20, 2009,
2. Request for additional Legal Descriptions, Exhibits and Associated Work
dated July 28, 2010
Original Contract Amount $24,024.00
Prior amendments $ 2,831.00
Current Amendment Amount $ 1.142.00
Amended Contract Amount $27,997.00
The remainder of the Agreement will remain unchanged by this Amendment. � /

Indiana Avenue Extension
City of Spokane Valley Capital Improvement Project No. 0112
This scope letter supersedes and replaces the previous letter dated July 27, 2010 due to
the addition of Task 4. The City of Spokane Valley requires additional services on the
Indiana Avenue Extension project.
The scope of services consists of providing staff, equipment and office calculations for
the following tasks:
Task 1— County ROW Legal Description and Exhibit
Provide a revised legal description and exhibit for the County RR ROW
acquisition. See attached exhibit. The City will provide an ACAD drawing of the
proposed ROW take.
Task 1 work shall be complete within 5 working days after the City gives Notice
to Proceed.
Task 2 — NW Corner Flora Rd & Mission Ave Legal Description and Exhibit
Provide legal descriptions and exhibit for the ROW take for Tax Parcel
45124.0203 at the NW corner of the Flora Road and Mission Avenue intersection.
See attached exhibit. The City will provide an ACAD drawing of the proposed
ROW take.
Task 2 work shall be complete within 10 working days after the City gives Notice
to Proceed.
Indiana Ave Extension #0112 1 Request for Additional Surveying Services
Task 3 — SE Corner Flora Rd & Mission Ave Legal Description and Exhibit
Provide legal descriptions and exhibit for the ROW take for Tax Parcel
55182.4201 at the SE corner of the Flora Road and Mission Avenue intersection.
See attached exhibit. The City will provide an ACAD drawing of the proposed
ROW take.
Task 3 work shall be complete within 10 working days after the City gives Notice
to Proceed.
Task 4 — NE Corner Flora Rd & Mission Ave Title Review
Provide a written opinion on ROW ownership abutting Tax Parcel 5 5073.3 101 at
the NE corner of the Flora Road and Mission Avenue intersection. The City has
emailed CLC the Title Report prepared by Spokane County Title Company.
Task 4 work shall be complete within 10 working days after the City gives Notice
to Proceed.
