TIRS AMENDMENT No. 4 is made to the Agreement for Professional Services dated
October 20, 2009 by and between the City of Spokane Valley, a code City of the State
of Washington, hereinafter "City" and CLC Associates, Inc., hereinafter "Consultant"
and jointly referred to as "parties."
Pursuant to Section 1.0 Modifications of the Agreement for Professional
Services, the parties hereby agree to amend the Agreement as set forth in the
attached 10/26 /10 COSV request for Easement Legal Descriptions and Exhibits
and the 10/29/10 COSV email for additional topographic survey. These services
will be tracked and billed on time and materials basis, and are subject to all of
the conditions set forth in the original Agreement. The City agrees to pay
Consultant up to $3,800.00 for the additional services provided for in this
Amendment to the Agreement.
Exhibits to this Amendment are as follows:
1. Professional Services Agreement dated October 20, 2009,
2. Request for additional Legal Descriptions, Exhibits and Associated Work
dated October 26, 2010
3. Email request for additional topographic survey dated Oct 29, 2010
Original Contract Amount $24,024.00
Prior amendments $ 3,923.00
Current Amendment Amount $ 3.800.00
Amended Contract Amount $31,747.00
The remainder of the Agreement will remain unchanged by this Amend C m nt.

The City of Spokane Valley requires additional services on the Indiana Avenue Extension
project.
The scope of services consists of providing staff, equipment and office calculations for
the following tasks:
Task 1 Border Easements - Legal Descriptions and Exhibits
Provide legal descriptions and exhibit for the border easements for Tax. Parcel
45131,0115 shown on the attached Preliminary Border Easement Plan.
Task 2 work shall be complete within 10 working days after the City gives Notice
to Proceed.

45131.0112 shown on the attached Preliminary Border Easement Plan.

Request for Additional Surveying Services
Provide legal descriptions and exhibit for the drainage easements for Tax Parcel
45131.0112 shown on the attached Preliminary Drainage Easement Plan.
Task 2 work shall be complete within 10 working days after the City gives Notice
to Proceed.

B Topo existing fence on north side of vacant lot.
C Topo trees and landscape area between vacant lot and residence from fence on west to existing edge of asphalt.
D Topo yard of Residence from edge of pavement to 10 feet west of property line
E Topo driveway from edge of asphalt to 10 feet west of property line
Task 2 1527 N Coach Road
A Topo north west corner of back yard
Provide ground shots at 10' o.c.; fence posts, top of slope; bottom of slope; and break lines.

Indiana Avenue Extension that we are currently processing.
