The Contract is revised as follows:
Description of Work
Schedule A — Barker Road Bridge
The Contractor shall perform core drilling of the Pier 3 Left and Pier 4 Right Exterior
shafts and testing as directed by the City and as specified in Section 6-19.3(9)H of the
Special Provisions.
Measurement
Section 6-19.4 of the Special Provisions is supplemented with the following:
No unit of measure shall apply to the lump sum price for "Drilled Shaft Coring
and Testing."
Payment
Section 6-19.5 of the Special Provisions is revised as follows:
Payment for "Drilled Shaft Coring and Testing" will be made in the lump sum
,amount of $34,907.75. The lump sum payment shall constitute full compensation
for all labor, materials, and equipment to complete the work as specified.
Working Days
The Contract time is unchanged.

Jim,
I've prepared the change order document for the Pier 3 Left and Pier 4 Right- Exterior drilled shaft coring
and testing work. Please review the attached document and return two signed copies if it is acceptable
to you. Otherwise, please let me know if you have any comments or concerns.
M &O was directed to perform the Pier 3 Left and Pier 4 Right- Exterior coring and testing work by our
March 24, 2009 and April 29, 2009 letters, respectively. According to Special Provision 6- 19.3(9)H, the
City is responsible to pay for the coring costs if no defect is found. Both shafts were found to be
acceptable based on the test results.
M &O submitted a cost summary for the coring work along with backup documentation in a letter to
CH2M HILL dated July 6, 2010. The City has reviewed this information and found it to be acceptable.
