Supplement Phase Design
Supplement Number 02
The Local Agency of City of Spokane Valley desires to supplement the agreement entered into with
CH2M HILL, Inc and executed on November 19, 2004. All provisions in the basic agreement remain
in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:
Section II, SCOPE OF WORK, is hereby amended as follows:
Provide additional surveying services necessary for utility agencies to design and relocate utilities as
required to accommodate the proposed street widening project. Additional surveying will be as
follows:
a. Compute and prepare coordinates for the following locations:
1) Back of sidewalk and back of right-of-way at existing power pole locations
2) Back of right-of-way at inter-visible locations in no power poles zones
3) Radius points or angle points of right-of-way at all intersections
b. Coordination with design team for design evaluations if required.
c. Setting hub and lath at each location with. appropriate designation.
2. Extend completion date, as specified in Section IV, to correspond with bidding process which will
occur during Winter 2005-2006.
Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the Completion Date
SUPPLEMENTAL COMPLETION DATE February 28, 2006
Section V, PAYMENT, shall be amended as follows as set forth in Exhibit A
