This Contract is revised as follows:
Description of Work
The Spokane Valley City Council approved a plan to stripe Broadway Avenue as a
3-lane section between Pines Road (SR 27) and Sullivan Road. The following
changes to the contract support this change:

Add Item: Spokane County Type M Curb, Doweled 164 LF
Curb construction shall conform to Spokane County Standard Plan A-3. Curb shall
be doweled into the existing pavement, 4' minimum spacing. Curb shall be installed
on planed asphalt surface.

Description of the Change:
This change order is being prepared to compensate the Contractor for additional work
required to:
1. Stripe Broadway as a 3-lane section between Pines and Sullivan.
2. Added Job Mix Compliance Price Adjustment and Compaction Price
Adjustment.
3. Added concrete curb and island at Summit School.
4. Vehicle detection system upgrades to accommodate the 3-lane section.
5. Repair a broken catch basin frame and grate.
The 3-lane striping change was approved by Spokane Valley City Council on June 13,
2006 (after contract award). The items Job Mix Compliance Price Adjustment and
Compaction Price Adjustment should have been included in the original bid documents.
The island at Summit School is a pedestrian safety improvement. The broken frame
was an existing condition. The grate was broken by traffic running on it during
construction.
Evolution of the Change:
Change Order Cover Memo
The 3-lane striping configuration was proposed by City staff and approved by the
City Council after contract award. Therefore the change order is required to
address the construction changes associated with this design.
Per an email from Bill McCammon at H8~LP the items Job Mix Compliance Price
Adjustment and Compaction Price Adjustment were added.
The need for the catch basin frame and grate repair work was discovered during
construction
Entitlement:
This work vas not included in the contract; Therefore, the Contractor could not
have anticipated nor bid this work. The Contractor is entitled to additional
compensation and time to perform this work.
Price:
The estimated total cost of this change is $41,747.18.
Contract Time:
14 additional wor#cing days will be added to the contract.
Grinding is complete. All of the work except for the striping must be completed prior to
paving. The 14 days was arrived at based on vehicle detector loop ordering time and
installation. The Contractor has agreed to this.
