Seope of Work Task 2.3.4: -inventory Forms Prepare historic and archaeological inventory survey forges in accordance with requirements of Section 106 and the Department of Archaeological and Historical Preservation Office (DAHP) for the Barker Road Bridge project. These inventories will be submitted to an online DAHP data base. Budget The estimated budget for the scope of work is 5:1.007.00. Schedule Dehverables are to be completed by _March 1, 2007. This letter outlines the scope of work and estimated fee associated with the additional effort to determine the appropriate layout for the future illumination system. This work was not included in our original level of effort estimate for the bridge final design phase. The provisions for the future bridge illumination system will include conduits, junction boxes, and mounting locations for the light poles. Light poles, luminaries, conductors, and electrical power metering and control system will not be provided as part of this project. Scope of Work CH.2M HILL will perform the following professional engineering services: • Coordinate with City staff to determine appropriate lighting criteria. • Select two types of luminaries to model. • Perform illuunination system modeling and calculations. • Determine required pole height and spacing necessary to satisfy identified lighting criteria. • Prepare a one to two page technical memorandum summarizing the lighting criteria, results of the modeling and calculations, pole height and spacing recommendations, and required conduit sizes. • Prepare final design drawings and specifications for the conduits and junction boxes to serve the future illumination system. CH2MHILL AXA41 4 F AMENDMENT # 05 -1 This amended AGREEMENT supercedes the original AGREEMENT between Washington State Department of Transportation and Cornell, Howland, Hayes and Merryfield Incorporated originally signed and dated by Donald K. Nelson on November 10, 1598 as subsequently amended. The original AGREEMENT has been and will be amended from time -to -time to reflect updated information. This is an amendment to the original AGREEMENT referenced above between Washington State Department of Transportation ( WSDOT) and Cornell, Howland, Hayes and Merryfield Incorporated (CH2M HILL) defining the Overhead Rate to be applied by CH2M HILL to all new WSDOT / CH2M HILL contracts effective per the dates shown herein. The following amended terms and conditions shall apply for the period of June 1, 2005 through May 31, 2008: • ON -CALL CONTRACTS' FORMULA: The Average hourly rate of all CH2M HILL direct labor hours will be calculated. The calculation will be made using the actual total direct labor hourly dollars and the actual total direct labor hours that were billed to WSDOT for all the previous (2004 In this case) calendar year's WSDOT / CH2M HILL "On-Call" contracts. • LUMP SUM AND COST PLUS FIXED FEE CONTRACTS' FORMULA: The average hourly rate of all CH2M HILL direct labor hours will be calculated. Using the above defined formulas and applying CH2M HILL's most current Computer, Telecommunications and Health & Safety Federal Rates, a single, SUPPLEMENTAL OVERHEAD RATE will be computed to cover these costs. This SUPPLEMENTAL OVERHEAD RATE will then be added to CH2M HILL's then current BASE OVERHEAD RATE thus creating a new overhead rate for all new CH2M HILL / WSDOT contracts executed under the terms of this amended AGREEMENT. SUPPLEMENTAL OVERHEAD RATE Details: A SUPPLEMENTAL OVERHEAD RATE to cover the computer, telecommunications and Health & Safety costs will be calculated and applied as described herein. MOOT OVERheAO RAZE AGREEMENT 40: -IDDG AMENDMENT # 05 -1 This SUPPLEMENTAL OVERHEAD RATE will not be subject to audit. Once the SUPPLEMENTAL OVERHEAD RATE Is applied and in effect for any CH2M HILL / WSDOT contract or agreement, it will not be adjusted for the life'of the contract, including any supplements, amendments, addendums, modifications, or new task orders. All supplements, amendments, and task orders will be priced and billed using the SUPPLEMENTAL OVERHEAD RATE in eff ect at the time the original contract was executed. The SUPPLEMENTAL OVERHEAD RATE will be re- evaluated annually on or about May 318' or as close to that date as possible for application to new CH2M HILL / WSDOT contracts (including their respective contract supplements, amendments, addendums, and modifications) to be executed under a future amended AGREEMENT. Re- evaluations that result In upward or downward modifications to the SUPPLEMENTAL OVERHEAD RATE, will be applied to new WSDOT / CH2M HILL fully executed contracts in accordance with the timeframe and terms of a new subsequent amended AGREEMENT. This Amendment #05 -1 describes the latest modification to the SUPPLEMENTAL OVERHEAD RATE and will be effective June 1, 2005 through May 31, 2006 as described in the above paragraph for new contracts executed in this timeframe. BASE OVERHEAD RATE Details: The BASE OVERHEAD RATE to cover direct and indirect costs associated with labor will be calculated and applied as described herein. n The BASE OVERHEAD RATE will be used for negotiation and billing purposes. It will be a provisional rate subject to audit and subsequently adjusted upward or downward to actual as required by specific CH2M HILL / WSDOT contracts, based on CH2M HILL's most current FAR Part 31- Compliant AUDITED OVERHEAD RATE. All contracts, supplements, amendments, and task orders will be priced and billed using the BASE OVERHEAD RATE in effect at the time the contract, supplement, amendment, or task order Is executed. The BASE OVERHEAD RATE will be re- evaluated and re- applied annually on or about May 31" based on CH2M HILL's most current FAR Part 31- Compliant AUDITED OVERHEAD RATE for pricing and billing new CH2M HILL / WSDOT contracts, task orders, supplements, and amendments to be executed under the timeframe and terms of a future amended AGREEMENT.
