Dear Mr. Worley:
We are providing this letter as our request to amend Task Number 001 of Agreement
A11102.000. Per your request, we have added the following items to the Description of Work:
4. Provide a report documenting the results of the past and current ratings for the bridge, a
summary of the results of the ratings, an explanation of options for posting the bridge, and
any additional recommendations we might have.
5. Coordinate with City, County, and WSDOT staff in determining the best course of action
for the bridge.
6. Additional minor load rating related work for the bridge.
To perform this work, we would like to increase the Maximum Amount Payable from
$3000 to $5000. We would also like to revise the Completion Date from 06/30/2011 to
09/30/2011. If all of these amendments to Task Number 001 are acceptable, please sign in the
space provided below and return a copy for our files via email, fax, or mail.
