This morning at 9:00 AM I directed Jeff Kerr of Murphy Brothers to lower the inverts of Storm Drain pipes SD), SD1, SD2
and SD3. See the attached portion of Sheet 10. This will allow the upstream invert of SD Pipe 0 to mesh with the new
ditch on the west side of Evergreen Rd.
Jeffs crew had already installed the last stick of SD3 and grouted it into existing Drywell 2. 1 agreed to pay for the
revised work by Force Account procedures that will include manhours and equipment cost to A) remove and relay SD 3 ;
brick and mortar the old core hole; place the new SD3 outfall in the body of Drywell 2; furnish and install 12 inch collars
at CBI, CB2 and CB 3.

On August 22, 2012, we uncovered a shallow phone line in the sub- grade. Ryan Kelly, our Project
Manager.began making phone calls to get ]t taken care of. The line was lowered to the proper depth on
August 27, 2.012 at 5:00 p:m. This slowed our operations and at times completely stopped'our trucks
from entering the job site i nd prevented our grade crews from working.
We are.requesting additional days 'be added to our contract because of this ynforoseen condition.
