For good and valuable consideration, the legal sufficiency of which is hereby acknowledged,
City and the Blews Construction Incorporated mutually agree as follows:
1. Purpose:
This Amendment is for the Contract for the Precinct Office Addition Project by and between the
Parties, executed by the Parties on July 16, 2013, and which terminates on August 2, 2013. Total
compensation under the Original Contract is not to exceed $13,127.70. Said contract shall be
referred to as the "Original Contract" and its terms are hereby incorporated by reference.
2. Original Contract Provisions:
The Parties agree to continue to abide by those terms and conditions of the Original Contract,
dated July 16, 2013, and any amendments thereto, which are not specifically modified by this
Amendment.
3. Amendment Provisions:
This Amendment is subject to the following amended provisions, which are as follows. All such
amendment provisions are hereby incorporated by reference herein and shall control over any
conflicting provisions of the Original Contract, including any previous amendments thereto.
Work completed date to be August 16 2013 rather than August 2 2013.
Change Order #1 — Add three receptacle outlets and reroute cubicle feed to south face of new
south wall Cost to be $385.00 plus $33.50 W S S T for a total cost of $418.50

4. Compensation Amendment History:
This is Amendment #1 of the Original Contract and the history of amendments to the
Consultant's / Contractor's compensation is as follows:

Total Amended Compensation $13,546.20
The parties have executed this Amendment to the Original Contract this day of August,
2013.
